摘 要:目的:观察肠内营养中添加益生菌对高龄老年患者营养状态及血脂的影响。方法:本研究为随机、单盲,选择≥80岁的高龄全肠内营养者为受试者,随机分为对照组(肠内营养)和干预组(益生菌+肠内营养)。分别测定试验开始时、1个月、2个月、3个月的血常规[白细胞(white blood cell,WBC)、血红蛋白(hemoglobin,Hb)、淋巴细胞绝对值(lymphocyte,L)、淋巴细胞百分比(lymphocyte%,L%)],营养相关检测[总蛋白(total protein,TP)、白蛋白(albumin,ALB)、前白蛋白(pro-albumin,pro-ALB)],脂质代谢血液学检测[三酰甘油(triglyceride,TG)、总胆固醇(total cholesterol,TC)、低密度脂蛋白胆固醇(low density lipoprotein cholesterol,LDL-C)、高密度脂蛋白胆固醇(high density lipoprotein cholesterol,HDL-C)],并观察患者相关临床事件的发生,包括消化道反应、大便性状、发热、抗生素使用情况等。结果:(1)治疗3个月后,两组WBC、Hb、L、L%差异均无统计学意义(均P>0.05)。两组血清TP、ALB、pro-ALB差异均有统计学意义(均P<0.05),干预组随着时间的越长,TP、ALB、pro-ALB增长越多(P<0.05)。(2)两组脂质代谢相关指标差异无统计学意义(P>0.05)。(3)两组间相关临床事件差异均无统计学意义(均P>0.05)。结论:联合益生菌的肠内营养方式可促进肠内营养的实施及支持效果,改善患者的营养状态。Objective:The study was to observe the effect of enteral nutrition containing probiotics in elderly patients on nutritional status and blood lipids.Methods:The study was a randomized,single blind design in sixty older participants aged 80 and over.Participants were randomized to a parallel group intervention of 3 months duration with a daily intake of enteral nutrition containing probiotics and enteral nutrition only.We evaluated whether probiotics administration alters blood parameters(blood routine examination such as white blood cell,hemoglobin,lymphocyte count,lymphocyte percentage and biochemical analysis such as total protein,albumin,pro-albumin,triglyceride,total cholesterol,low density lipoprotein cholesterol,high density lipoprotein cholesterol)and observed the occurrence of some clinical events(abdominal distension,diarrhea,gastric retention,fever,use of antibiotic)in different time points(the begin of the study,first month,second month,third month).Results:(1)There was no statistically significant difference between two groups in blood routine including white blood cell,lymphocyte count,lymphocyte percentage and hemoglobin concentration after 3 months treatment(P>0.05).Serum total protein,albumin and prealbumin increased compared with the control group(P<0.05),and in probiotic group,gradually increasing with time.(2)There was no significant difference in blood lipids between the control group and the probiotic group(P>0.05).(3)The occurrence of related clinical events between two groups was not statistically significant difference(P>0.05).Conclusions:The enteral nutrition combined with probiotics can promote the implementation and support effect of enteral nutrition and improve the nutritional status of patients.
