摘　　要：目的:探讨超声辐射力脉冲成像(ARFI)技术评价脑卒中痉挛性瘫痪患者小腿腓肠肌内侧头及比目鱼肌的价值。方法:选取临床确诊为脑卒中并单侧肢体痉挛性瘫痪患者42例,常规超声测量肌肉厚度及羽状角。ARFI技术测量踝关节自然位及最大背伸位肌肉组织的声触诊组织定量(VTQ)值,并行统计学分析。结果:脑卒中患者患侧腓肠肌内侧头及比目鱼肌的肌肉厚度和羽状角均较其健侧减小(均P<0.05);健侧及患侧腓肠肌内侧头的最大背伸位VTQ值均大于自然位,健侧及患侧比目鱼肌最大背伸位VTQ值均大于自然位(均P<0.05)。相同体位下,脑卒中患者患侧的VTQ值高于健侧,差异有统计学意义(均P<0.05)。结论:超声ARFI技术能定量客观评价脑卒中痉挛性瘫痪患者的肌肉参数,为临床提供新的评价方法。Objective:To explore the value of acoustic radiation force impulse(ARFI)in evaluating the medial head of calf gastrocnemius and soleus in spastic paralysis patients with stroke.Methods:42 patients with stroke and spastic paralysis of one side limbs were selected.The muscle thickness and feather angle were measured by conventional ultrasound.ARFI technique was used to measure the virtual touch tissue quantification(VTQ)value of ankle joint in natural position and maximum dorsiflexion,and the results were statistically analyzed.Results:The muscle thickness and feather angle of the medial head of gastrocnemius and soleus in diseased side were significantly smaller than those in the healthy side(both P<0.05).The VTQ values of gastrocnemius and soleus in maximum dorsiflexion in both sides were higher than those in natural position,the differences were statistically significant(both P<0.05).Under the same body position,the VTQ values of the affected side were higher than those of healthy side,the differences were statistically significant(both P<0.05).Conclusion:Ultrasound ARFI can evaluate the muscle parameters of spastic stroke patients quantitatively and objectively,and provide a new method for clinical evaluation.
