临床治疗联合多重干预用于社区2型糖尿病综合管理分析

摘　　要：目的探讨临床治疗联合多重干预对社区2型糖尿病综合管理的效果。方法选取医院2018年11月至2019年11月收治的社区2型糖尿病患者144例,第1阶段为6个月,按随机数字表法分为观察组及对照组,各72例。两组均予常规治疗,观察组患者予6个月小组组长教育,对照组患者予6个月大组健康教育。第2阶段为进行到第8个月时,入选72例,分为常规治疗组和多重干预组,各36例,比较分析两组的腰臀比、糖化血红蛋白(HbA1C)、体质量指数等情况。结果两组患者的第1阶段及第2阶段的腰臀比、年龄、HbA1C、体质量指数无显著差异(P>0.05);在同伴支持小组的作用下,在第1阶段干预结束后,相较于基线,HbA1C无显著差异。相较于基线,对照组HbA1C有显著差异;第2阶段干预结束后,相较于多重干预组,第8个月、13个月的腰臀比、HbA1C、体质量指数无显著差异(P>0.05)。结论针对社区2型糖尿病患者,采用6个月小组健康教育,以及常规治疗,并联合多重干预,可改善患者自我管理行为、疾病有关状态,加快康复速度。Objective To investigate the effect of clinical treatment combined with multiple interventions on the comprehensive management of patients with type 2 diabetes in community.Methods Totally 144 patients with type 2 diabetes in the community were selected from November 2018 to November 2019.The first stage was 6 months,72 patients were divided into the observation group and the control group according to the method of random number table,the patients in the observation group were educated by the group leader for 6 months,and the patients in the control group were educated by the health education for 6 months.In the eighth month,the second stage started,72 cases were selected and divided into the conventional treatment group and the multiple intervention group,36 cases in each group.The waist to hip ratio,HbA1C and BMI of the two groups were compared and analyzed.Results There were no significant differences in waist to hip ratio,age,HbA1C and BMI between the two groups at the first and second stages(P>0.05).Under the influence of peer support group,there was no significant difference in HbA1C compared with baseline after the intervention at the first stage.Compared with baseline,there was significant difference in HbA1C in the control group.After the second stage of intervention,there was no significant difference in waist to hip ratio,HbA1C and BMI at the 8th and 13th month between the multiple intervention group and the conventional treatment group(P>0.05).Conclusion For patients with type 2 diabetes in the community,6-month group health education,routine treatment and multiple interventions can improve self-management behavior,disease-related status and speed up rehabilitation.
