摘 要:目的比较虎红平板凝集试验(RBT)、酶联免疫吸附试验(ELISA)和试管凝集试验(SAT)等血清学检测方法的特性和诊断效能,为科学检测提供数据依据。方法同时利用RBT、ELISA和SAT等方法检测221份血液标本,对阳性率进行比较。以细菌培养为金标准,绘制ROC曲线,分析诊断效能。结果 SAT的阳性率均低于RBT和ELISA,RBT与ELISA的阳性率差异无统计学意义(P=0. 590)。RBT、ELISA和SAT的AUC均在0. 7~0. 9,诊断效能均为中等。SAT的AUC、特异度、准确度、Youden指数、阳性似然比等指标均高于RBT和ELISA。ELISA和RBT的灵敏度较高,高于SAT。结论根据大规模初筛和门诊就诊病例的不同情况,综合利用RBT、ELISA和SAT等检测方法,有助于提高效率和准确度。Objective The research aimed to compare and analyze serological tests including RBT,SAT and ELISA,so as to provide a reference for scientific testing.Methods A total of 221 blood specimens were tested by RBT,SAT and ELISA respectively,and positive rates were compared.The gold standard was bacterial culture,and ROC curve was drawn to analyze the diagnostic efficiency.Results The positive rate of SAT was lower than that of RBT and ELISA.There was no statistical significance on the differences between the positive rates of RBT and ELISA(P=0.590).The AUC of RBT,ELISA and SAT were all within 0.7-0.9,and the diagnostic efficacies were all at medium.The AUC,specificity,accuracy,Youden Index and positive likelihood ratio of SAT were higher than these of RBT and ELISA.Both the sensitivities of ELISA and RBT were higher than that of SAT.Conclusion RBT,SAT and ELISA should be used synthetically to improve efficiency and accuracy,depending on different situation such as mass screening of cases and outpatients.
