摘 要:目的探讨GDM患者外周血中CD4+Foxp3+调节性T细胞的表达与血清25羟维生素D[25(OH)D]水平的关系。方法选取2018年10月至2019年3月于遵义医科大学附属医院内分泌科、产科门诊及住院患者120例,分为GDM组(孕24~28周)45例,正常妊娠(孕24~28周)对照组(GNGT组)45名,非妊娠单纯T2DM患者(T2DM组)30例。采用流式细胞术检测外周血中CD4+Foxp3+调节性T细胞的表达,电化学发光法测定25(OH)D水平。结果 GNGT、T2DM及GDM组血清25(OH)D水平依次降低[(36.11±5.85)vs(20.92±5.81)vs(10.74±6.48)μg/L,P<0.05]。血清25(OH)D与外周血中CD4+Foxp3+调节性T细胞占CD4+T细胞比例及HDL-C呈正相关(r=0.384、0.496,P<0.05),与FIns、HbA1c、HOMA-IR呈负相关(r=―0.583、―0.657、―0.492,P<0.05)。结论 GDM患者血清25(OH)D水平及外周血中CD4+Foxp3+调节性T细胞表达降低可能与GDM的发生发展有关。Objective To investigate the correlation between the expression of CD4+Foxp3+ regulatory T cells and serum 25-hydroxyvitamin D[25(OH)D] levels in peripheral blood of patients with gestational diabetes mellitus(GDM).Methods A total of 120 outpatients and inpatients from Endocrinology Department,Obstetrics Department in Affiliated Hospital of Zunyi Medical University were selected in this study from October 2018 to March 2019.All the patients were divided into GDM group(24~28 weeks gestation,n=45),normal pregnancy group(GNGT)(24~28 weeks gestation,n=45),non pregnancy T2DM group(T2DM group,n=30).The expression of CD4+Foxp3+ regulatory T cells in peripheral blood was detected by flow cytometry.The level of 25(OH)D was determined by electrochemiluminesce.The levels of 25(OH)D decreased in the three groups:GNGT group(36.11±5.85)μg/L vs T2DM group(20.92±5.81)μg/L vs GDM group(10.74±6.48)μg/L(P<0.05).Serum 25(OH)D was positively correlated with the ratio of CD4+Foxp3+ regulatory T cells in peripheral blood to CD4+T cells and HDL-C(r=0.384,0.496,P<0.05),while negatively correlated with FIns,HbA1c,and HOMA-IR(r=―0.583,―0.657,―0.492,P<0.05 or P<0.01).Conclusion The decreased expression of CD4+Foxp3+ regulatory T cells and serum 25(OH)D in peripheral blood of patients with GDM may be related to the occurrence and development of GDM.
