针灸联合清热拔毒药贴治疗湿热型膝骨关节炎疗效观察

摘　　要：目的:研究针灸联合清热拔毒药贴治疗湿热型膝骨关节炎患者的临床疗效。方法:选取符合要求90例患者,随机分为针灸联合清热拔毒药贴组(联合组)、针灸安慰组(针灸组)和西药组,联合组和针灸组均给予相同针灸穴位,联合组针灸后加清热拔毒药贴2h,5d为1个疗程,共2个疗程,西药组予以布洛芬消炎止痛治疗10d。骨关节炎指数评估量表(WOMAC)、视觉模拟量表(VAS)、肿瘤坏死因子α(TNF-α)、白细胞介素-1β(IL-1β)、基质金属蛋白酶(MMP-1)。结果:治疗前3组各项指标比较差异均无统计学意义,治疗后联合组在关节功能恢复和疼痛改善方面均优于其他两组(P<0.05),对于TNF-α、IL-1β、MMP-1炎症改善,联合组和西药组较为接近,都优于针灸组(P<0.05)。结论:针灸联合清热拔毒药贴能缓解湿热型膝骨关节炎患者疼痛,改善膝关节功能,值得在临床上推广与应用。Objective:To study the clinical effect of acupuncture and moxibustion combined with heat-clearing and toxin-removing patch in the treatment of damp-heat knee osteoarthritis.Methods:Ninety patients who met the experimental requirements were randomly divided into acupuncture and moxibustion combined with heat-clearing and toxin-removing patch group(A),acupuncture and comfort group(B)and western medicine group(C).Both groups A and B were given the same acupuncture points.Group A was treated with heat-clearing and toxin-removing patch for 2 hours after acupuncture and moxibustion,and 5 days was a course of treatment.There were two courses of treatment.Group C was treated with ibuprofen for anti-inflammation and analgesia.WOMAC index,VAS score,TNF-α,IL-1βand MMP-1 were compared before and after treatment.Results:There was no statistical difference in the indexes before treatment.After treatment,the improvement of joint function and pain in group A was better than that in group B and C(P<0.05),and the improvement of inflammation of TNF-α,IL-1βand MMP-1 in group A was similar to group C,but better than group B(P<0.05).Conclusion:Acupuncture and moxibustion combined with heat-clearing and toxin-removing patch can relieve pain and improve knee function in patients with damp-heat knee osteoarthritis,which is worthy of clinical promotion and application.
