摘　　要：目的探讨反晕征(RHS)在不同肺部疾病中的特征及其与相关疾病的关系,以提高鉴别诊断能力。资料与方法收集肺部CT图像中可见RHS病灶的28例患者,分析其影像学和临床资料。结果①诊断:28例患者中,肺结核12例,隐源性机化性肺炎9例,真菌感染4例,肺腺癌2例,肺栓塞1例。②征象:12例肺结核RHS内可见结节,16例RHS病灶内未见结节。9例隐源性机化性肺炎病灶具有反复、游走性的特点。1例肺腺癌RHS见于放射治疗后。③病灶数量:单发RHS病灶7例,多发RHS病灶21例。④伴发征象:除RHS外,其他影像表现包括树芽征12例、微结节影15例、磨玻璃影10例、纤维条索影7例、实变影15例、小叶间隔增厚9例、空洞征1例、胸膜改变22例、胸腔积液4例等。结论RHS可见于多种疾病,但在不同肺部疾病中具有各自的特点,结合其他肺部CT征象及相关临床资料,可有助于临床鉴别诊断。Purpose To explore the characteristics of reversed halo sign(RHS)in different lung diseases and its relationship with related diseases to improve the ability of differential diagnosis.Materials and Methods Twenty-eight patients with RHS lesions seen in CT images of the lungs were enrolled in this study,the imaging and clinical data were also analyzed.Results(1)Diagnosis:among the 28 patients,12 were tuberculosis,9 were cryptogenic organizing pneumonia,4 were fungal infection,2 were lung adenocarcinoma and 1 was pulmonary embolism.(2)Signs:nodules were visible in the RHS of 12 tuberculosis,while nodules were not seen in other 16 cases.The focal point of 9 cryptogenic pneumonitis was of the recurrent and wandering characteristics.One case of lung adenocarcinoma showed the RHS after radiotherapy.(3)The number of RHS lesions:there were 7 cases of single lesion and 21 cases of multiple lesions.(4)Accompanying signs:besides RHS,other visible image signs including tree-in-bud(12 cases),micro-nodule(15 cases),ground-glass opacity(10 cases),thin reticular reticulation(7 cases),consolidation(15 cases),interlobular septal thickening(9 cases),cavity(1 cases),pleural change(22 cases),pleural effusion(4 cases)were also founded.Conclusion RHS can be found in a variety of diseases,but has its own characteristics in different lung diseases.Therefore,RHS can provide certain help for clinic diagnosis and differential diagnosis when it is combined with other lung CT signs and related clinical data.
