丹参川芎嗪注射液治疗慢性心力衰竭血瘀证患者的临床研究

摘　　要：目的观察丹参川芎嗪注射液对慢性心力衰竭血瘀证患者的临床疗效并探讨其作用机制。方法70例患者随机分为治疗组及对照组各35例。对照组予常规的抗心衰治疗,治疗组在对照组的基础上联合丹参川芎嗪注射液。观察两组患者的中医证候疗效、心功能分级、血浆N末端B型钠尿肽前体(NT-proBNP)、左室射血分数(LVEF)、心肌能量消耗(MEE)水平及生活质量(MLHFQ评分)的变化情况。结果治疗后,治疗组中医证候疗效总有效率80.00%高于对照组的57.14%(P<0.05);治疗组心功能分级总有效率85.71%高于对照组的77.14%(P<0.05)。两组治疗前NT-pro BNP、LVEF及MEE比较,差别均不大(均P>0.05)。治疗后,两组NTproBNP、LVEF及MEE与治疗前比较均改善,且治疗组较对照组更为显著(均P<0.05)。两组治疗前MLHFQ评分比较,差别不大(P>0.05)。治疗后两组MLHFQ评分与治疗前比较均降低,且治疗组生活质量评分低于对照组(P<0.05)。结论常规治疗联合丹参川芎嗪注射液可改善慢性心衰血瘀证患者临床症状、减轻心肌能量消耗、提高生活质量。Objective:To study the clinical effect of Danshen Ligustrazine Injection on patients with blood stasis syndrome of chronic heart failure. Methods:70 patients were randomly divided into the treatment group and the control group,35 cases in each group. The control group was given conventional anti-heart failure treatment.Based on it,the treatment group was treated with Danshen Ligustrazine Injection. The changes of TCM syndrome efficacy,cardiac function classification,plasma N-terminal B-type natriuretic peptide precursor,left ventricular ejection fraction,myocardial energy expenditure and quality of life were observed. Results:After treatment,the total effective rate of TCM syndromes in the treatment group was 80.00%,significantly higher than that in the control group(57.14%)(P < 0.05);The total effective rate of the cardiac function classification in the treatment group was85.71%,higher than that of the control group(77.14%)(P < 0.05). There was no significant difference in NT-pro BNP,LVEF and MEE between the two groups before treatment(P > 0.05). After treatment,they were improved,and the treatment group was significantly better than the control group(P < 0.05). There was no significant difference in MLHFQ between the two groups before treatment(P > 0.05). After treatment,MLHFQ scores of the two groups decreased,and the quality of life score of the treatment group was lower than that of the control group(P <0.05). Conclusion:Conventional treatment combined with Danshen Ligustrazine Injection can significantly improve the clinical symptoms,reduce myocardial energy consumption and improve the quality of life of patients with chronic heart failure and blood stasis syndrome.
