摘 要:目的观察红光照射联合七叶一枝花酊外用治疗蝮蛇咬伤的临床疗效,探讨中西医联合干预在蝮蛇咬伤救治中的优势。方法收集200例蝮蛇咬伤患者,采用随机数字表法分为对照组及试验组各100例。对照组采用西医常规疗法;试验组在对照组基础上红光照射及七叶一枝花酊外用。两组共治疗6 d。观察并分析两组患者的局部症状(疼痛程度、肿胀程度)、炎性指标[中性粒细胞计数百分比(N%)、C反应蛋白(CRP)、白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、转化生长因子β1(TGF-β1)]的变化情况。结果两组治疗前后患肢肿胀和疼痛情况均有统计学差异(P<0.05),但对照组在第3天与治疗前比无明显差异(P>0.05),而试验组有明显差异(P<0.05)。两组相比,试验组患者在治疗第3、6天的局部症状与对照组相比差异均有统计学意义(P<0.05)。治疗后试验组N%、CRP、TGF-β1改善情况较对照组优(P<0.05),在TNF-α、IL-6上两组治疗结果相当,无显著差异(P>0.05)。结论红光照射联合七叶一枝花酊外用治疗蝮蛇咬伤具有更显著的临床治疗效果,更能有效缓解患肢疼痛及肿胀。Objective:To observe the clinical effect of red light irradiation combined with Qiyeyizhihua Tincture on viper bite patients,and discuss the advantages of combined Chinese and Western therapy for pit viper bite.Methods:200 cases were randomly divide into the control group and the experimental group,100 cases in each group.The control group received conventional Western therapy.The experimental group was treated with red light irradiation combined with Qiyeyizhihua Tincture on that basis.The two groups were treated for 6 days.Local symptoms(pain level,swelling level)and inflammatory indicators(N%,CRP,IL-6,TNF-αand TGF-β1)were observed and analyzed in both groups.Results:There were significant differences in swelling and pain between the two groups before and after treatment(P<0.05).However,there was no significant difference in the control group between the pre-treatment and the third day(P>0.05),while there was significant difference in the experimental group(P<0.05).Compared with the control group,the local symptoms of the patients in the experimental group were statistically significant on the 3 rd and 6 th days(P<0.05).The improvement of N%,CRP and TGF-β1 in the experimental group was better than those in the control group(P<0.05),and there were no significant differences in TNF-αand IL-6 between the two group(P>0.05).Conclusion:Red light irradiation combined with Qiyeyizhihua Tincture has more significant clinical therapeutic effect on pit viper bite,and it is more effective in relieving pain and swelling of affected limb.
