清热凉血解毒方对大鼠痔术后创面愈合及炎症因子表达的影响

摘　　要：目的探讨清热凉血解毒方对大鼠痔术后创面愈合的作用及对相关炎症因子表达的影响。方法 50只SD大鼠随机分为空白对照组、模型组、清热凉血解毒方低、中、高剂量组,每组10只,除空白对照组外,其他各组采用肛周局部注射冰醋酸法建立大鼠痔术后创面感染模型,造模成功后第2天空白对照组和模型组给予生理盐水灌胃,清热凉血解毒方低、中、高剂量组给予相应剂量的清热凉血解毒汤灌胃,剂量均为10 mL/kg,每日1次,连续7 d。治疗7 d后比较各组大鼠水肿程度和创面面积,肛周病理组织,采用ELISA法检测血清肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)和白介素-8(IL-8)水平,采用免疫组化法检测肛周组织血管内皮生长因子(VEGF)和前列腺素E2(PGE2)的表达。结果与空白对照组比较,模型组水肿程度评分和创面面积显著高于对照组,清热凉血解毒方各剂量组水肿程度评分和创面面积显著低于模型组,且中、高剂量组低于低剂量组(P <0.05)。清热凉血解毒方各剂量组肛周组织间隙疏松,水肿不明显,少量鳞状上皮坏死,少量中性粒细胞,毛细血管增生不明显。与空白对照组比较,模型组TNF-α、IL-6和IL-8水平显著升高,清热凉血解毒方各剂量组TNF-α、IL-6和IL-8水平显著低于模型组,且中、高剂量组低于低剂量组(P <0.05)。与空白对照组比较,模型组VEGF和PGE2水平显著升高,清热凉血解毒方各剂量组VEGF和PGE2水平显著低于模型组,且中、高剂量组低于低剂量组(P <0.05)。结论清热凉血解毒方可有效促进大鼠痔术后创面愈合,改善肛周组织,降低炎症反应,其机制可能与降低VEGF和PGE2水平有关。Objective:To investigate the effect of Qingre Liangxue Jiedu Decoction on wound healing in rats with hemorrhoids after operation and the effect on the expression of related inflammatory factors. Methods:50 SD rats were randomly divided into the control group,the model group,low-dose,medium-dose and high-dose groups of Qingre Liangxue Jiedu Decoction,10 rats in each group. Except the control group,other groups were established with local anal injection of glacial acetic acid. The control group and the model group were given intragastric gavage on the 2 nd day after the model was successfully established,and Qingre Liangxue Jiedu Decoction groups were given gavage with corresponding dose,once a day for 7 days. After 7 days′ treatment,the edema degree,wound area and perianal pathological tissue of rats in each group were compared. The levels of TNF-α,IL-6 and IL-8 were detected with ELISA;the expressions of VEGF and PGE2 in perianal tissue were detected with immunohistochemical method. Results:Compared with the control group,the edema degree score and wound area of the model group were significantly higher,and those of Qingre Liangxue Jiedu Decoction groups were significantly lower than those of the model group,and the middle and high dose groups were lower than low dose group(P < 0.05).The perianal tissue space was loose;edema was not obvious with a small number of squamous epithelial necrosis and neutrophils;capillary hyperplasia was not obvious in Qingre Liangxue Jiedu Decoction groups. Compared with the control group,the levels of TNF-α,IL-6,IL-8,VEGF and PGE2 in the model group rose significantly,and those of Qingre Liangxue Jiedu Decoction groups were significantly lower than those of the model group,and the middle and high dose groups were lower than low dose group(P < 0.05). Conclusion:Qingre Liangxue Jiedu Decoction can effectively promote the wound healing of hemorrhoids in rats,improve the perianal tissue and reduce the inflammatory response.
