摘  要:目的探讨DR负相翻转技术在尘肺病诊断中的应用优势,为提高基层职业健康检查机构在尘肺病诊断中的准确率提供影像学诊断依据.方法回顾性选择98例诊断为疑似职业性尘肺病的胸部DR平片,分别应用DR负相翻转技术阅片和普通阅片方法进行影像学诊断,对2种诊断方法得出的诊断结果进行统计学对比分析.结果DR负相翻转技术对p影、1级密集度、右下小阴影、左下小阴影分布范围的判定均比普通阅片有较高准确率,χ^2值分别为4.86、4.23、5.12、5.88,差异有统计学意义(P<0.05);2种方法对尘肺病的分期诊断一致性中等(k=0.59,P<0.01);DR负相翻转技术对尘肺病诊断的准确度、敏感度、特异度分别为93.87%、95.34%、83.33%,均明显高于普通阅片的65.31%、69.77%、33.3%,差异有统计学意义(P<0.001).结论DR负相翻转技术在尘肺病诊断中的准确度、敏感度、特异度较普通阅片高,具有明显的优势,值得在基层职业健康检查机构中推广应用.Objective To explore the advantages of DR negative phase inversion technique and provide imaging diagnostic basis of pneumoconiosis in primary ocupational health examination organizations.Methods 98 patients who were suspected with occupational pneumoconiosis were included in this study.The imaging diagnosis were performed by DR negative phase inversion technique and ordinary image respectively.Two groups of diagnostie results were analyzed by X^2 test analysis and Kappa test analysis.Results The DR negative phase inversion technique had higher accuracy than ordinary images,in determination of lesion distribution range,such as p-shadow,grade 1 density,right-lower and left-lower small shadows。in which X^2 values were 4.86.4.23,5.12.5.88 respectively(P<0.05).The two methods in staging pneumoconiosis had moderate consistency(k=0.59,P<0.01).The accuracy,sensitivity and specificity of DR negative phase inversion technique in diagnosis of pneumoconiosis were 93.87%,95.34%and 83.33%respectively,which were higher than those of ordinary images with statistical significance(65.31%,69.77%,33.39%,P<0.001).Conclusion The accuracy.sensitivity and specificity of DR negative phase inversion technique in diagnosis of pneumoconiosis are higher than those of ordinary images.It has obvious advantages and is worthy for popular applying in primary occupational health examination organizations.
