临床药师参与病区药房管理对药师医嘱审核能力的影响研究

摘　　要：目的分析临床药师参与病区药房管理对药师医嘱审核能力的影响。方法从首都医科大学附属北京同仁医院南区医院信息系统收集病区药房2015年9月1日至2016年8月31日、2017年9月1日至2018年8月31日间医嘱审核数以及不合理医嘱干预有关资料,进行回顾性分析,统计分析临床药师参与病区药房管理前后不合理医嘱数量的变化。结果临床药师参与病区药房管理后,干预不合理用药医嘱由99例上升到254例,干预率由原来的0.01%上升到0.03%,用法用量不适宜115例,占比45.3%,其它类型(药物选择不适宜、重复给药、给药途径不适宜、不良相互作用等)139例,占比54.7%;临床药师参与病区药房管理前,用法用量不适宜61例,占比61.6%,其它类型(药物选择不适宜、重复给药、给药途径不适宜、不良相互作用等)38例,占比38.4%。结论临床药师归属病区药房参与医嘱审核,有利于提高药学整体水平,保证病人用药安全。Objective To analyze the influence of clinical pharmacists involved in management of inpatient pharmacy on the improvement of pharmacists’ability to review medical orders.Methods The number of medical order audits and relevant information on unreasonable medical order intervention of the ward pharmacy were collected from the information system of the Southern District Hospital of Beijing Tongren Hospital affiliated to Capital Medical University from September 1,2015 to August 31,2016,and from September 1,2017 to August 31,2018.Results The number of intervention records of irrational medical orders increased from 99 cases to 254 cases and intervention rate increased from 0.01% to 0.03%,115 cases(45.3%)of inappropriate usage and dosage and 139 cases(54.7%)of other types(inappropriate drug selection,repeated drug administration,inappropriate drug delivery route and adverse interactions)after clinical pharmacists involved in management of inpatient pharmacy;before clinical pharmacists involved in management of inpatient pharmacy,61 cases(61.6%)of inappropriate usage and dosage and 38 cases(38.4%)of other types.Conclusion Clinical pharmacists participate in the inpatient pharmacy management and review of medical order,can conductively improve the overall level of pharmacy and ensure patient medication safety.
