摘　　要：目的探讨构建家庭医生签约服务运行模式对社区居民就诊行为的影响。方法选取2017~2018年广州市海珠区构建家庭医生签约服务的8个社区共2584名居民为研究对象并进行问卷调查。问卷内容含居民一般情况资料和就诊行为问卷两个部分,就诊行为内容包括就诊医疗机构选择情况和原因、通过社区医疗机构的转诊情况、转诊方式、家庭医生签约的意愿情况等项目共7个一级指标和59个二级指标。结果家庭医生签约者的社区医疗机构就诊率和通过社区医疗机构的转诊率均高于同年未签约者,2018年签约者的社区医疗中心的转介率高于2017年,2017年未签约居民选择社区医疗机构的就诊率高于2018年,差异均有统计学意义(P<0.05)。结论家庭医生签约服务运行模式能改善社区居民就诊行为,提高居民对于家庭医生签约服务的认识。Objective To explore the influence of family doctor contract service operation mode on community residents’ medical treatment behavior. Methods A total of 2584 residents of construct family doctor contract service from 8 communities in Haizhu district of Guangzhou city were selected as the study objects and questionnaires were conducted from 2017 to 2018.The questionnaire including general information of residents and medical treatment behavior. The contents of medical treatment behavior were 7 primary indicators and 59 secondary indicators, including the selection and reasons of medical institutions, the referral status through community medical institutions, the methods of referral, and the willingness of family doctors to sign up.Results The community medical institutions outpatient rates and the referral rates through community medical institutions of the family doctor contractor were higher than those of unsigned in the same year(P<0.05),the community health center referral rates of contractor in 2018 were higher than those in 2017(P<0.05), the community medical institutions outpatient rates of unsigned residents in 2017 were higher than those in 2018(P<0.05). Conclusion The family doctor contract service can improve the medical treatment behavior of community residents and enhance their understanding of family doctor contract service.
