摘 要:目的探讨智能盆底超声联合多平面成像技术在产后压力性尿失禁(SUI)筛查中的应用。方法选取2016年9月至2018年12月在本院收治90例SUI产后产妇作为SUI组,同期选取90例非SUI的健康产后产妇作为健康组,所有产妇均行静息和Valsalva动作状态下智能盆底超声联合多平面成像技术检查。结果SUI组静息和Valsalva动作状态下膀胱尿道后角、盆膈裂孔面积明显高于健康组,SUI组静息和Valsalva动作状态下耻骨直肠肌厚度明显低于健康组,SUI组膀胱颈移动度、尿道旋转度、尿道内口漏斗发生率明显高于健康组,差异有统计学意义(P<0.05),SUI组和健康组尿道长度、尿道倾斜角比较,差异无统计学意义(P>0.05);智能盆底超声联合多平面成像技术筛查SUI的敏感度、特异度、准确度为90.00%、93.33%、91.67%,与实际结果基本一致(P<0.05)。结论智能盆底超声联合多平面成像技术可有效显示SUI产妇的膀胱颈和尿道支持结构改变,在SUI筛查中具有良好的应用价值,值得临床推广。Objective To discuss the application of intelligent pelvic floor ultrasound combined with multiplanar imaging in screening of postpartum stress urinary incontinence(SUI).Methods From September 2016 to December 2018,90 SUI postpartum women in our hospital were selected as SUI group,and 90 non-SUI healthy postpartum women were selected as health group during the same period,all parturients was given pelvic floor ultrasound combined with multiplanar imaging under resting and Valsalva motion.Results The area of posterior horn and pelvic diaphragm hole of bladder and urethra in SUI group was significantly higher than that in healthy group at rest and Valsalva action,the thickness of puborectalis muscle in SUI group was significantly lower than that in healthy group at rest and Valsalva action,the incidence of bladder neck mobility,urethral rotation and urethral funnel in SUI group was significantly higher than that in healthy group,the difference was statistically significant(P<0.05),there was no significant difference in urethral length and urethral obliquity between SUI group and healthy group(P>0.05).The sensitivity,specificity and accuracy of intelligent pelvic floor ultrasound combined with multiplanar imaging in screening SUI were 90.00%,93.33%and 91.67%,which were basically consistent with the actual results(P<0.05).Conclusions Intelligent pelvic floor ultrasound combined with multi-planar imaging technology can effectively display the changes of bladder neck and urethral support structure in postpartum women with SUI,which has the good application value in SIU screening,it's worth for further clinical promotion.
