理筋正骨手法联合中药热敷治疗神经根型颈椎病临床观察

摘　　要：目的观察理筋正骨手法联合中药热敷对慢性颈椎病患者肌电图、临床疗效的影响。方法选择我院收治的神经根型颈椎病患者84例,随机分为对照组和观察组各42例,对照组予以理筋正骨手法治疗,观察组在对照组治疗基础上联合中药热敷治疗,共治疗5周;比较两组症状体征量化评分、临床疗效、肌电图变化和疼痛评分。结果治疗后,观察组治疗总有效率显著高于对照组(P<0.05);观察组症状体征量化评分、正中神经和尺神经F波出现率、传导速度显著高于对照组,现在疼痛状况(PPI)、疼痛分级指数(PRI)和视觉模拟评分(VAS)评分分别显著低于对照组(P<0.05)。结论理筋正骨手法联合中药热敷治疗神经根型颈椎病疗效较好,可有效减轻患者症状和疼痛感,并改善神经根压迫。Objective To observe the effect of tendon-regulation and bone-setting manipulation combined with traditional Chinese medicine(TCM)fomentation on electromyogram(EMG)of patients with chronic cervical spondylosis.Methods 84 cases of patients with cervical spondylosis were randomly divided into control group and observation group,42 cases each.Control group was treated with tendon-regulation and bone-setting manipulation,observation group was treated with TCM fomentation on the basis of control group.Both groups were treated for 5 weeks.The quantitative scores of symptoms and signs,clinical curative effect,EMG changes and pain score were compared between the two groups.Results After treatment,the total response rate of observation group was significantly higher than that of control group(P<0.05);the quantitative scores of symptoms and signs,appearance rate of median nerve and ulnar nerve F wave,and conduction velocity in observation group were significantly higher than those in control group;the PPI,PRI and VAS scores were significantly lower than those in control group(P<0.05).Conclusion The curative effect of tendon-regulation and bone-setting manipulation combined with TCM fomentation is relatively better on cervical spondylosis,which can effectively alleviate symptoms and pain of patients,and improve nerve root compression.
