摘 要:目的运用循证医学的方法探讨精索静脉曲张与精子DNA损伤之间的关系。方法计算机检索Cochrane Database of Systematic Reviews、PubMed、MEDLINE、CNKI、EMbase、万方等数据库中关于精索静脉曲张与精子DNA损伤之间关系的相关研究,同时追索相关临床研究的数据库和纳入文献的参考文献,扩大检索范围,获取所有相关文献。文献检索时间截止至2019年9月20日。制定严格的纳入和剔除标准,依据Cocharane手册质量评价标准对纳入的文献进行质量评价。采用Review Manager5.3软件进行meta分析。结果共纳入8篇文献,共有精索静脉曲张患者220例,未患有精索静脉曲张者181例。Meta分析结果显示:精索静脉曲张患者与对照组对比,精子DNA损伤率明显高于对照组(P<0.01),Mean Difference(MD)值14.51%,95%CI为(6.90,22.13),P=0.0002。结论与未患有精索静脉曲张组比较,精索静脉曲张组有明显高的精子DNA损伤率。Objective The Meta-analysis was conducted to evaluate the relationship between varicocele and sperm DNA damage.Methods The literatures were searched from several databases without language restriction,which included Cochrane Database of Systematic Reviews,PubMed,MEDLINE,CNKI,EMbase,WanFang and other databases on the relationship between varicocele and semen DNA,as well as a database of relevant clinical studies and references for inclusion in the literature,expand the scope of the search,and obtain as much as possible of all relevant literature.The search time limit is built until September 20,2019.Strict inclusion and rejection criteria were developed and the quality of the included literature was evaluated according to the Cocharane Manual Quality Assessment Criteria.Meta-analysis was performed using Review Manager 5.3 software.Results There were 8 studies involving 220 patients with varicoceles,and 81 without varicoceles were recruited.The estimate showed that patients with varicoceles have significantly higher sperm DNA damage than controls(P<0.01),with a mean difference of 14.51%(95%CI 6.90 to 22.13;P=0.0002).Conclusion Compared with the group without varicocele,the varicocele group had a significantly higher rate of semen DNA damage.
