摘  要:目的分析99mTc-DTPA肾动态显像对静脉肾盂造影(IVP)不显影患者肾功能的评估效果。方法于2016年1月-2019年1月平顶山市第二人民医院收治的IVP不显影患者中选取80例,所有患者均给予99mTc-DTPA肾动态显像,采集患者的肾动态显像图像并测定其GFR值。结果 80例IVP不显影患者经99mTc-DTPA动态显像后,患肾显影率达到了72.5%,其中0级占比2.5%,GFR值为(51.6±2.3)mL/min,1级占比5.0%,GFR值为(36.2±3.4)mL/min,2级占比7.5%,GFR值为(23.5±2.9)mL/min,3级所占比重为57.5%,GFR值为(14.3±3.6)mL/min,4级占比27.5%,GFR值为(7.5±2.1)mL/min。肾功能重度以上受损所占比重达到了85.0%,经患肾切除、保留肾脏手术等治疗后,所有患者术后肾功能均恢复良好。结论 99mTc-DTPA肾动态显像在肾功能评价中具有更高的准确性和灵敏性,能够较为准确地评估IVP不显影患者的患肾功能,从而为临床手术方式选择和是否保留患肾功能提供重要参考依据。Objective To analyze the effect of dynamic renal imaging with 99 mtc-dtpa on evaluating renal function in patients without IVP imaging. Methods From January 2016 to January 2019, totally 80 non-developing IVP patients admitted to the Second People’s Hospital of Pingdingshan City were selected. All patients were given 99 mtc-dtpa renal dynamic imaging, and their renal dynamic imaging images were collected and their GFR values were determined.Results After dynamic imaging of 80 IVP patients without IVP development, the renal development rate reached 72.5%, of which grade 0 accounted for 2.5%, GFR value was(51.6±2.3) mL/min, grade 1 accounted for 5.0%, GFR value was(36.2±3.4) mL/min, grade 2 accounted for 7.5%, GFR value was(23.5±2.9) mL/min, grade 3 accounted for 57.5%, GFR value was(14.3±3.6) mL/min, and grade 4 accounted for 27.5%. The GFR value was(7.5±2.1) mL/min. The proportion of severely impaired renal function reached 85.0%. After nephrectomy, nephrectomy and nephrectomy, all the patients recovered well. Conclusion The dynamic renal imaging of 99 ctc-dtpa has higher accuracy and sensitivity in the evaluation of renal function, and can accurately evaluate the renal function of patients without IVP imaging, thus providing an important reference for the selection of clinical surgical methods and retention of renal function.
