摘 要:目的:探讨外周血细胞膜脂肪酸C20∶1n9水平与冠状动脉病变严重程度的关系。方法:选取2018年1月至2018年12月期间,我院行冠状动脉造影及心导管检查的200例患者作为研究对象,根据造影结果分为冠心病组(n=151)和非冠心病组(n=49),比较两组患者的一般资料,及外周血细胞膜脂肪酸C20∶1n9水平。多因素Logistic回归分析C20∶1n9水平与冠心病的相关性。计算Gensini积分以评估冠心病患者冠状动脉病变严重程度。采用Sperman相关性分析外周血红细胞膜脂肪酸C20∶1n9水平与冠心病患者冠状动脉病变严重程度Gensini积分相关性。结果:冠心病组患者年龄、男性比、糖尿病比例以及红细胞膜脂肪酸C20∶1n9比例均显著高于非冠心病组患者(P<0.05),不同严重程度的冠心病患者外周血红细胞膜脂肪酸C20∶1n9数据比较,差异有统计学意义(F=7.0743,P=0.000),Spearman相关分析结果显示外周血红细胞膜脂肪酸C20∶1n9比例与冠心病Gensini积分存在正相关(r=0.285,P=0.000),多因素Logistic回归分析结果显示性别、年龄、糖尿病以及红细胞膜脂肪酸C20∶1n9比例是冠心病的危险因素。结论:高水平的外周血红细胞膜脂肪酸C20∶1n9是冠心病的危险因素,可能作为冠心病及冠状动脉病变严重程度的预测因子。Objective:To investigate the relationship between the level of membrane fatty acid C20:1n9 in peripheral blood and the severity of coronary artery disease.Methods:200 patients undergoing coronary angiography and cardiac catheterization from January 2018 to December 2018 were divided into coronary heart disease group(n=151)and non-coronary heart disease group(n=49).The general data and the level of erythrocyte membrane fatty acid C20:1n9 of the two groups were compared.Multivariate Logistic regression analysis was used to analyze the correlation between C20:1n9 level and coronary heart disease.Gensini score was calculated to evaluate the severity of coronary artery disease.The correlation between C20:1n9 level of erythrocyte membrane fatty acid and Gensini score of severity of coronary heart disease was analyzed by Spearman correlation analysis.Results:The age,male ratio,diabetes mellitus ratio and erythrocyte membrane fatty acid C20:1n9 ratio in coronary heart disease group were significantly higher than those in non-coronary heart disease group(P<0.05).There were significant differences in erythrocyte membrane fatty acid C20:1n9 data among patients with different severity of coronary heart disease(F=7.074,P=0.000).The result of Spearman correlation analysis showed that there was a positive correlation between the proportion of fatty acid C20:1n9 in peripheral blood erythrocyte membrane and Gensini score of coronary artery disease(r=0.285,P=0.000).Multivariate Logistic regression analysis showed that there was a positive correlation between fatty acid ratio and Gensini score in peripheral blood erythrocyte membrane.Conclusions:high level of erythrocyte membrane fatty acid C20:1n9 in peripheral blood is a risk factor for coronary heart disease and may be a prerequisite for the severity of coronary heart disease and coronary artery disease.Measure the factor.
