超声引导下中心静脉导管穿刺治疗子宫切口愈合不良的临床价值

摘　　要：目的探讨采用超声引导下中心静脉导管穿刺引流治疗剖宫产术后子宫切口愈合不良的临床疗效,为临床有效治疗子宫切口愈合不良提供新的思路。方法随机选择2012年1月—2018年12月在山东省聊城市人民医院进行子宫下段剖宫产术,子宫切口愈合不良患者66例,其中试验组31例行超声引导下中心静脉导管穿刺引流,对照组35例行常规保守治疗,观察两组患者体温恢复正常时间、住院时间和包块消退时间之间的差异。结果一般临床资料和临床指标的监测,试验组与对照组相比较,差异无统计学意义(P>0.05);治疗效果上试验组产妇体温恢复正常时间、住院时间、包块消失时间分别为(6.6±0.9)d、(10.2±1.4)d、(18.7±4.7)d,对照组产妇分别为(7.2±1.6)d、(12.1±2.8)d、(21.8±5.4)d,试验组与对照组比较,差异有统计学意义(t=3.039、4.001、3.018,P<0.05)。结论超声引导下中心静脉导管穿刺引流在治疗子宫切口愈合不良中是一种安全、有效、可行的方式。Objective To investigate the clinical efficacy of ultrasound-guided central venous catheter puncture and drainage in the treatment of poor uterine incision healing after cesarean section,and to provide new ideas for clinically effective treatment of poor uterine incision healing.Methods A total of 66 patients with poor uterine incision healing were randon performed at Liaocheng People's Hospital of Shandong Province from January 2012 to December 2018.Of the 31 patients in the experimental group,ultrasound-guided central venous catheter puncture and drainage were performed.35 patients in the control group were treated with conventional conservative treatment,and the differences between the time when the temperature of the two groups returned to normal,the length of hospital stay,and the time of mass subsidence were observed.Results The clinical data and clinical indicators were monitored.Compared the control group with the experimental group(P>0.05),and there was no statistical difference between the two groups.In terms of treatment effect,the time for the maternal body temperature of the test group to return to normal,the length of hospital stay,and the time of mass loss were(6.6±0.9)d,(10.2±1.4)d,(18.7±4.7)d,and those in the control group were(7.2±1.6)d,(12.1±2.8)d,and(21.8±5.4)d.Compared with the control group,the difference was statistically significant(t=3.039,4.001,3.018,P<0.05)..Conclusion Ultrasound-guided central venous catheter puncture and drainage is a safe,effective and feasible method in the treatment of poor healing of uterine incision,and it is worthy of clinical application.
