左旋布比卡因行腰-硬联合麻醉于下肢骨科手术麻醉的作用探讨

摘　　要：目的系统研究左旋布比卡因行腰-硬联合麻醉应用在下肢骨科手术麻醉中的作用与价值,为手术治疗提供有价值的参考依据。方法随机将2017年12月-2018年12月方便选取该院50例行下肢骨科手术患者分为实验组(25例,应用腰-硬联合麻醉,15 min后呈仰卧体位)、对照组(25例,应用2 mL浓度为0.75%布比卡因+1 mL浓度10%葡萄糖注射液单次注射)。对比两组患者麻醉效果、阻滞前后MBP(平均动脉血压)、HR(心率)。结果实验组麻醉总有效率96%(24/25)远远高于对照组72%(18/25),临床对比差异有统计学意义(χ~2=5.357,P<0.05);阻滞前,实验组平均动脉血压为(93.12±10.01)mmHg、心率为(81.44±11.26)次/min,对照组平均动脉血压为(93.14±10.03)mmHg、心率为(81.45±11.24)次/min,组间差异无统计学意义(t=0.007、0.003,P>0.05);阻滞后,实验组平均动脉血压为(92.02±8.86)mmHg、心率为(77.89±11.24)次/min,对照组平均动脉血压为(82.87±8.02)mmHg、心率为(77.89±11.24)次/min,实验组均优于对照组,差异有统计学意义(t=3.828、2.260,P<0.05)。结论左旋布比卡因行腰-硬联合麻醉应用于下肢骨科手术麻醉中,麻醉效果确切,可使接受下肢骨科手术患者疼痛感明显缓解,为手术治疗提供必要保障,可在下肢骨科手术中推广应用。Objective To systematically study the role and value of levobupivacaine combined with lumbar-hard anesthesia in the anesthesia of lower extremity orthopedic surgery, and provide a valuable reference for surgical treatment. Methods Convenient select From December 2017 to December 2018, 50 patients with lower extremity orthopedic surgery in our hospital were randomly divided into experimental group(25 cases, combined with lumbar-hard anesthesia, 15 minutes after supine position) and control group(25 cases, apply 2 mL of a concentration of 0.75% bupivacaine +1 mL of a 10% glucose injection to a single injection). The anesthetic effect of the two groups of patients, MBP(mean arterial blood pressure) and HR(heart rate) before and after the block were compared. Results The total effective rate of anesthesia in the experimental group 96%(24/25) was much higher than that in the control group 72%( 18/25). There was a statistically significant difference in clinical comparison(χ~2=5.357,P<0.05). Before the block, the experimental group averaged. The arterial blood pressure was(93.12±10.01) mmHg and the heart rate was(81.44±11.26) times/min. The mean arterial blood pressure of the control group was(93.14±10.03) mmHg, and the heart rate was(81.45±11.24) times/min. The difference between the groups was not statistically significant. The comparison between groups showed(t=0.007,0.003,P>0.05);the lag was delayed. The mean arterial blood pressure of the experimental group was(92.02±8.86) mmHg, the heart rate was(77.89±11.24) times/min,and the mean arterial blood pressure of the control group was(82.87±8.02)mmHg. The heart rate was(83.54±12.06)times/min. The experimental group was superior to the control group, and the difference was statistically significant(t =3.828,2.260,P<0.05). Conclusion L-bupivacaine combined with lumbar-rigid anesthesia is used in lower limb orthopaedic anesthesia.
