摘　　要：系统性接触性皮炎是一种变态反应性皮肤病。经皮肤接触致敏的人通过口服、肌肉注射、静脉输注、吸入或经皮肤黏膜吸收等途径再次接触相同半抗原时,可能会发生系统性接触性皮炎。系统性接触性皮炎的临床表现多种多样,既往接触性皮炎部位或斑贴试验阳性部位皮炎复发是较为常见而特异的表现,还可表现为瘙痒性水疱性手部湿疹、屈侧皮炎、狒狒综合征和泛发性斑丘疹等。药物是引起系统性接触性皮炎的最常见原因,其他原因还包括金属如镍、钴、铬、汞、金和铝等,食品添加剂如秘鲁香脂、山梨酸和甜味剂等。目前对系统性接触性皮炎的发病机制还了解较少,除了经典的T细胞介导的迟发超敏反应外,速发超敏反应以及多种细胞因子也有可能参与发病。当明确了系统性接触性皮炎的原因后,避免系统接触是最重要的防治方法。Systemic contact dermatitis is an allergic inflammatory skin disease that may occur in persons with contact allergy when they are exposed to the hapten by oral,intramuscular,intravenous infusion,inhalation,or absorption through the skin and mucosa.The clinical manifestations of systemic contact dermatitis are various.The recurrence of dermatitis in previous contact dermatitis sites or patch test positive sites is a common and specific manifestation.It can also manifest as pruritic vesicular hand eczema,flexor dermatitis,Baboon syndrome and generalized maculopapular rash.Drugs are the most common cause of systemic contact dermatitis.Other causes include metal such as nickel,cobalt,chromium,mercury,gold,and aluminum;food additives such as balsam of Peru,sorbic acid,and sweeteners.The pathogenesis of systemic contact dermatitis is still poorly understood.In addition to the classic T cell-mediated delayed hypersensitivity,immediate hypersensitivity and a variety of cytokines may also be involved in the pathogenesis.When the cause of systemic contact dermatitis is identified,avoiding systemic contact is the most important prevention method.
