改良B-Lynch缝合术治疗剖宫产术后难治性产后出血的临床疗效分析

摘　　要：目的探讨改良B-Lynch缝合术治疗剖宫产术后难治性产后出血的临床疗效。方法方便选取2017年10月—2019年4月该院收治的110例剖宫产术后难治性产后出血患者,依据不同的研究方式将其分为对照组(n=55)与试验组(n=55),对照组施以宫腔填塞纱布方式治疗,试验组施以改良B-Lynch缝合术治疗,观察两组患者的临床治疗效果、术中出血量、术后48 h出血量、输血量、血红蛋白量情况、并发症发生情况、止血时间情况。结果试验组的临床治疗总有效率为96.36%,明显高于对照组的86.64%,差异有统计学意义(χ2=4.950,P=0.026);术中出血量、术后48 h出血量、输血量明显较少,血红蛋白量明显较高,止血时间明显较短,与对照组相比差异有统计学意义(P<0.05);并发症发生率为3.64%,明显低于对照组的16.36%,差异有统计学意义(χ2=4.950,P=0.026)。结论改良B-Lynch缝合术治疗剖宫产术后难治性产后出血具有显著的临床效果。Objective To study the clinical effect of modified B-Lynch suture in the treatment of refractory postpartum hemorrhage after cesarean section.Methods Selection in October 2017 to April 2019 the hospital 110 cases of patients with intractable convenient postpartum hemorrhage after cesarean section,on the basis of different research,it can be divided into the control group(n=55)and experimental group(n=55)and control group with uterine tamponade gauze treatment,the treatment of patients with improved B-Lynch suture,observe the clinical therapeutic effect of two groups of patients,intraoperative blood loss,postoperative 48 h blood loss,blood transfusion amount,quantity of hemoglobin,complications,the bleeding time.Results The total effective rate of clinical treatment in the experimental group was 96.36%,which was significantly higher than that in the control group 86.64%(χ2=4.950,P=0.026),and the amount of intraoperative bleeding,48 hours after operation,the amount of blood transfusion was much less,the amount of hemoglobin was higher and the time of bleeding was significantly shorter than those in the control group(P<0.05).The incidence of complications was 3.64%,which was significantly lower than that of the control group 16.36%(χ2=4.950,P=0.026).Conclusion Modified B-Lynch suture is used for the treatment of sutures.Treatment of refractory postpartum hemorrhage after cesarean section has significant clinical effect.
