补肾活血汤在治疗股骨头坏死的作用机制研究

摘　　要：目的观察补肾活血汤在治疗股骨头坏死中的临床疗效和作用机制。方法方便选择的研究对象为2017年6月—2018年6月的40例在该院接受治疗股骨头坏死患者。将上述患者按照治疗方式的不同分为观察组20例和对照组20例。两组患者的治疗均采用经皮多通道减压术来进行。对照组加用骨肽片配合治疗,观察组采用补肾活血汤加减配合治疗,持续治疗1个月为1疗程,治疗3个疗程后评定和对比两组患者治疗的临床效果和治疗前后的髋关节功能评定。结果治疗3个月后,观察组有显效8例,有效10例,无效1例,总有效率90.0%;对照组有显效5例,有效6例,无效4例,总有效率为75.0%;两组相比,总有效率之间的差异有统计学意义(χ2=4.124,P=0.034);治疗前,两组患者髋关节疼痛与活动度评分比较,差异无统计学意义(P>0.05);治疗3个疗程后,观察组髋关节疼痛评分与活动度评分与对照组相比,观察组均明显高于对照组,差异有统计学意义(t=3.075、3.275,P=0.000,0.035)。结论在治疗早中期股骨头缺血坏死的过程中,利用补肾活血汤来缓解股骨头坏死的症状,能够对患者的关节活动能力加以提高,能够对病情加以控制,并且能够延缓股骨头的坏死过程,在这些方面都有着较为显著的效果。Objective To observe the clinical effect and mechanism of Bushen Huoxue decoction in the treatment of femoral head necrosis.Methods The convenient selected subjects were 40 patients with femoral head necrosis treated in the hospital from June 2017 to June 2018.The above patients were divided into observation group and control group according to different treatment methods.Patients in both groups were treated with percutaneous multichannel decompression.The control group was treated with bone peptide tablets,and the observation group was treated with Bushen Huoxue decoction plus or minus treatment.The continuous treatment lasted for one course of treatment for 1 month.After 3 courses of treatment,the clinical effect of the two groups and the hip function evaluation before and after treatment were evaluated and compared.Results After 3 months of treatment,the observation group had obvious effect in 8 cases,10 cases,1 case,the total effective rate was 90.0%.In the control group,there were 5 cases with obvious effect,6 cases with effective effect,4 cases without effective effect,and the total effective rate was 75.0%.Compared with the two groups,the total effective rate was statistically significantly different(χ2=4.124,P=0.034).Before treatment,there was no statistically significant difference in hip pain and mobility scores between the two groups(P>0.05).After 3 courses of treatment,hip pain score and range of motion score in the observation group were significantly higher than those in the control group,with statistically significant differences(t=3.075,3.275,P=0.000,0.035).Conclusion In the treatment of avascular necrosis of femoral head in early and middle stages,Bushen Huoxue decoction can alleviate the symptoms of avascular necrosis of femoral head,improve the joint activity ability of patients,control the disease,and delay the process of avascular necrosis of femoral head.
