人工流产后流产不全的超声检查分析

摘　　要：目的探讨超声诊断人工流产后流产不全的临床价值。方法方便择取2016年1月—2018年9月于该院接受人工流产的176例患者为研究对象,全部患者流产后均以彩超复查,评价流产效果,统计流产不全率,观察超声声像图表现。结果超声诊断流产不全86例,检出率48.86%,基于金标准,流产不全诊断率100.00%,患者主要声像图表现为宫腔内不均质回声区,与子宫肌层边界清晰,内部及周边有明显血流,可见低阻频谱信号,阻力指数<0.6。其余患者中,46例超声复查示子宫内膜回声均匀,23例示子宫内膜回声不均匀,15例回声尚均匀,但宫腔线分离,内有形状不规则条索状无回声区,或见细密光点,但多普勒与频谱检查均未探及明显血流信号与频谱信号,6例宫腔内不均质回声。结论超声检查可以有效检出人工流产后宫腔内妊娠组织残留,联合多普勒与频谱超声检查明确血流与频谱信号情况,能够为患者是否需要再次清宫提供有价值的依据。Objective To explore the clinical value of ultrasound in the diagnosis of incomplete abortion after induced abortion.Methods 176 patients who received induced abortion in the hospital from January 2016 to September 2018 were convenient selected as the research objects.All patients were reexamined by color Doppler ultrasound after abortion to evaluate the effect of abortion,to count the rate of incomplete abortion,and to observe the sonographic manifestations.Results Ultrasound diagnosis of 86 cases of incomplete abortion,the detection rate was 48.86%.Based on the gold standard,the diagnosis rate of incomplete abortion was 100.00%.The main sonographic features of the patients were inhomogeneous echo area in uterine cavity,clear boundary with uterine muscular layer,obvious blood flow in and around the uterus,low resistance spectrum signal and resistance index less than 0.6.Among the remaining patients,46 cases showed homogeneous endometrial echoes,23 cases showed heterogeneous echoes in endometrial functional areas,15 cases showed homogeneous echoes in functional areas,but the line of uterine cavity was separated,there were irregular strips and cords without echoes,or fine spots of light,but both Doppler and spectral examinations were detectable.Obvious blood flow signal and spectrum signal,6 cases showed heterogeneous echoes in uterine cavity.Conclusion Ultrasound can effectively detect intrauterine pregnancy tissue residues after induced abortion.Combined with Doppler and spectrum ultrasound,blood flow and spectrum signals can be clearly identified,which can provide valuable basis for patients whether they need to re-clear the uterus.
