养正消积胶囊联合化疗治疗非小细胞肺癌的疗效及安全性的Meta分析

摘　　要：目的系统评价养正消积胶囊联合化疗治疗非小细胞肺癌的疗效及安全性。方法检索中国知网、Pubmed等7个数据库,对符合纳入标准的随机对照试验进行资料提取,参照Cochrane质量评价标准评价文献质量,采用Revman 5.3软件进行Meta分析。结果最终纳入11项研究,合计1 075例患者。Meta分析结果显示,试验组近期有效率[RR=1.35,95%CI(1.17,1.57),P<0.000 1]、疾病控制率[RR=1.16,95%CI(1.08,1.25),P<0.001]、生活质量改善率[RR=1.92,95%CI(1.39,2.65),P<0.000 1]均显著高于对照组。不良反应中试验组白细胞减少[RR=0.59,95%CI(0.50,0.70),P<0.000 01]、血小板减少[RR=0.52,95%CI(0.42,0.64),P<0.000 01]、血红蛋白减少[RR=0.44,95%CI(0.22,0.86),P=0.02]、胃肠道反应[RR=0.62,95%CI(0.49,0.77),P<0.000 1]均低于对照组。结论养正消积胶囊联合化疗治疗非小细胞肺癌效果优于单纯化疗,并可减轻化疗毒副反应。但由于纳入研究的局限性,有待更多高质量临床研究进一步验证。Objective To assess the efficacy and safety of Yangzheng Xiaoji capsule combined with chemotherapy in the treatment of non-small cell lung cancer. Methods A systematic literature search was performed in 7 databases:CNKI, Wan Fang, CBM, VIP, Pubmed, Embase and The Cochrane Library, from database establishment to August,2019. The data were extracted from the randomized controlled trials(RCTs)that met the inclusion criteria,the quality of the literature referred to the Cochrane quality evaluation standard,and the review was performed using Review Manager 5.3 software. Results A total of 11 RCTs involving 1 075 patients were included. Meta-analysis results showed that the short-term effective rate[RR =1.35,95% CI(1.17,1.57),P < 0.000 1],disease control rate[RR=1.16,95% CI(1.08,1.25),P<0.001],and quality of life improvement rate[RR =1.92,95% CI(1.39,2.65), P<0.000 1] in the experimental group were significantly higher than those in the control group, with statistically significant differences(P < 0.05). Adverse reactions in the experimental group,leukopenia decrease[RR=0.59,95% CI(0.50,0.70),P<0.000 01],thrombocytopenia decrease[RR=0.52,95% CI(0.42,0.64),P<0.000 01],decreased hemoglobin[RR=0.44,95% CI(0.22,0.86),P = 0.02],gastrointestinal adverse reactions [RR=0.62,95% CI(0.49,0.77),P<0.000 1]were all lower than those of the control group,and the difference were statistically significant(P < 0.05). Conclusion The results of this study showed that the effect of Yangzheng Xiaoji capsule combined with chemotherapy in the treatment of non-small cell lung cancer was better than that of chemotherapy alone,and the adverse reactions of chemotherapy could be alleviated. However,due to the limitations of the included studies,more high-quality clinical trials are needed for further verification.
