摘　　要：目的探讨可溶性程序性死亡分子配体1(sPD-L1)、结核菌素纯蛋白衍生物(TB-PPD)及结核感染T细胞斑点试验(T-SPOT.TB)联合检测诊断菌阴肺结核的临床价值。方法选取菌阴肺结核患者50例为研究组,另选取非结核感染的肺病患者50例为常规组。均进行sPD-L1、TB-PPD及T-SPOT.TB联合检测诊断。结果研究组sPD-L1高于常规组(P<0.05);联合检测诊断特异性、敏感性、ROC曲线下面积及约登指数均优于一种检测指标。结论联合sPD-L1、TBPPD及T-SPOT.TB指标检测诊断菌阴肺结核的临床价值更高。【Objective】To investigate the clinical value of soluble programmed death-ligand 1(sPD-L1),purified protein derivative of tuberculin(TB-PPD)and T cell spot test of tuberculosis(T-SPOT.TB)in the diagnosis of sputum tuberculosis.【Methods】Fifty patients with sputum tuberculosis were selected as the study group,and 50 patients with non-tuberculosis infected lung disease were selected as the routine group.The combined detection of sPD-L1,TB-PPD and T-SPOT.TB was performed.【Results】The sPD-L1 of the study group was higher than the conventional group(P<0.05).The specificity,sensitivity,area under the ROC curve,and the Yoden index of the combined detection were better than those of single detection.【Conclusion】The clinical value of combined detection of sPD-L1,TB-PPD and T-SPOT.TB indicators for the diagnosis of sputum tuberculosis is higher.
