摘　　要：目的:分析原发性干燥综合征患者外周血调节性B细胞(Bregs)表达水平及临床意义。方法:选取蚌埠医学院第一附属医院2017年1月至2019年4月收治的36例诊断明确的原发性干燥综合征患者作为病例组,同时选取30例同时期门诊健康体检者作为对照组,采用流式细胞术测定对照组以及病例组治疗前、治疗4周后外周血Bregs细胞水平;采用ELISA法测定对照组以及病例组治疗前、治疗4周后血清白介素-10(IL-10)、转化生长因子β1(TGF-β1)水平,分析病例组治疗前外周血Bregs细胞水平与IL-10和TGF-β1水平的相关性,并比较不同临床特征患者外周血Bregs细胞水平。结果:病例组外周血Bregs细胞水平和血清IL-10、TGF-β1水平均显著高于对照组(P<0.05);抗SSA抗体、抗SSB抗体阳性患者外周血Bregs细胞水平均显著高于抗SSA抗体、抗SSB抗体阴性患者(P<0.05);治疗4周后,干燥综合征患者外周血Bregs细胞水平和血清IL-10、TGF-β1水平均显著低于治疗前(P<0.01);治疗前干燥综合征患者外周血Bregs细胞水平与血清IL-10、TGF-β1水平均呈正相关(r=0.589、0.004,P<0.05)。结论:原发性干燥综合征患者外周血Bregs细胞水平显著升高,可能与原发性干燥综合征的发病有一定关系,但与干燥综合征的疾病活动无直接相关性。Objective:To analyze the expression and significance of the regulatory B cells(Bregs)in peripheral blood of patients with primary Sjogren’s syndrome.Methods:A total of 36 patients with primary Sjogren’s syndrome admitted to the First Affiliated Hospital of Bengbu Medical College from Jan 2017 to Apr 2019 were selected as the case group,and 30 healthy outpatients in the same period were selected as the control group.Flow cytometry was used to detect the expression of Bregs in peripheral blood of control group and case group before and after 4 weeks of treatment,and the serum levels of interleukin-10(IL-10)and transforming growth factor-β1(TGF-β1)were measured by ELISA.The correlation of Bregs with the levels of IL-10 and TGF-β1 before treatment in the case group was analyzed with Spearman.The levels of Bregs in peripheral blood of patients with different clinical characteristics were compared.Results:The levels of peripheral blood Bregs,serum IL-10 and TGF-β1 in the case group were significantly higher than those in the control group(P<0.05).The levels of peripheral blood Bregs in patients with positive anti-SSA and anti-SSB antibodies were significantly higher than those in patients with negative anti-SSA and anti-SSB antibodies(P<0.05).After 4 weeks of treatment,the levels of Bregs,IL-10 and TGF-β1 in the case group were significantly lower than those before treatment(P<0.01).There was a significant positive correlation between the expression of peripheral blood Bregs and the serum levels of IL-10 and TGF-βin patients with Sjogren’s syndrome before treatment(r=0.589,0.004,P<0.05).Conclusion:The expression level of Bregs in peripheral blood of patients with primary Sjogren’s syndrome increases significantly,which may be related to the pathogenesis of the disease,but not directly related to the disease activity.
