鸦胆子油乳注射液联合化疗治疗中晚期结直肠癌疗效和安全性的Meta分析

摘　　要：目的:系统评价鸦胆子油乳注射液联合化疗治疗中晚期结直肠癌的有效性和安全性。方法:计算机检索CNKI、WanFang Data、PubMed、Cochrane Central、EMbase数据库,搜集鸦胆子油乳注射液联合化疗治疗中晚期结直肠癌的随机对照试验(RCT),检索时限均从建库至2018年10月27日。由2位研究者独立进行文献筛选和资料提取,并评价纳入研究的偏倚风险,采用RevMan 5.3软件进行数据分析。结果:共纳入16个RCT,1432例中晚期结直肠癌患者。Meta分析结果显示:两组治疗中晚期结直肠癌的临床疗效、生活质量疗效、生活质量评分、体力状况比较结果分别为[RR=1.30,95%CI(1.15,1.46),P<0.000,1]、[RR=1.31,95%CI(1.13,1.53),P=0.000,5]、[MD=5.61,95%CI(4.68,6.55),P<0.000,01]、[MD=8.06,95%CI(4.43,11.68),P<0.000,1],表明鸦胆子油乳注射液联合化疗治疗中晚期结直肠癌优于单纯化疗组。且治疗组白细胞减少、恶心呕吐、骨髓抑制、粒细胞减少等不良反应发生率均低于对照组(P<0.05)。结论:鸦胆子油乳注射液联合化疗组与单纯化疗组相比,可以提高临床疗效,改善生活质量,减少不良反应。由于纳入研究数量和质量的不足,上述结论仍需要开展更多高质量的RCT予以证实。Objective:To systematically evaluate the efficacy and safety of Yadanzi Youru injection combined with chemotherapy in the treatment of advanced colorectal cancer.Methods:The computer searched CNKI,WanFang Data,PubMed,Cochrane Central,EMbase databases,and collected randomized controlled trials(RCT)of Yadanzi Youru injection combined with chemotherapy in the treatment of advanced colorectal cancer.The search time was the establishment of the database to October 27,2018.Two researchers independently conducted literature screening and data extraction,and evaluated the risk of bias in the included studies,using RevMan5.3 software for data analysis.Results:A total of 16 RCTs were included,and 1432 patients with advanced colorectal cancer were included.Meta-analysis results showed that the clinical efficacy,quality of life efficacy,quality of life score,and physical status of the two groups of patients with advanced colorectal cancer were[RR=1.30,95%CI(1.15,1.46),P<0.000,1],[RR=1.31,95%CI(1.13,1.53),P=0.000,5],[MD=5.61,95%CI(4.68,6.55),P<0.000,01],[MD=8.06,95%CI(4.43,11.68),P<0.000,1],indicating that the treatment of mid-advanced colorectal cancer with Yadanzi Youru injection combined with chemotherapy is superior to chemotherapy alone.Conclusion:Compared with the chemotherapy group,the Yadanzi Youru injection combined with chemotherapy group can effectively improve the clinical efficacy,improve quality of life,and reduce adverse reactions.Due to the insufficient quantity and quality of the included studies,the above conclusions still need to be confirmed by conducting more high-quality RCTs.
