俯卧位通气治疗急性呼吸窘迫综合征的疗效观察

摘　　要：目的探究俯卧位通气(PPV)治疗急性呼吸窘迫综合征(ARDS)的疗效。方法选取2015年12月-2019年6月收治的37例ARDS患者临床资料进行分析,均实行PPV治疗,比较治疗前后血流动力学与血气分析指标,并观察治疗后并发症情况。结果与治疗前比较,治疗2 h MAP(84.59±7.54)mm Hg、HR(92.65±16.74)次/分钟、CVP(11.41±2.10)cm H2O,p H(7.40±0.09)、Pa CO2(50.16±12.55)mm Hg,差异无统计学意义(P>0.05);治疗后Pa O2(123.50±55.54)mm Hg、Pa O2/Fi O2(196.63±55.54)mm Hg,较治疗前明显改善,差异有统计学意义(P<0.05);治疗后血压下降6.66%,皮肤黏膜损伤0.00%,呕吐胃内容物8.33%。结论ARDS患者予以PPV治疗有助于患者动脉氧合情况改善,且对血流动力学无显著不良影响,严重并发症发生率低。Objective To explore the curative effect of prone position ventilation(PPV)in the treatment of acute respiratory distress syndrome(ARDS).Methods The clinical data of 37 patients with ARDS admitted in our hospital between December 2015 and June 2019 were analyzed.All patients were treated with PPV.The haemodynamics and blood gas analysis indexes were compared before and after treatment,and the complications after treatment were observed and compared.Results At 2 hours after treatment,there were no significant changes in MAP(84.59±7.54)mm Hg,HR(92.65±16.74)times/min,CVP(11.41±2.10)cm H2O,p H(7.40±0.09),Pa CO2(50.16±12.55)mm Hg(P>0.05),but the differences in the Pa O2(123.50±55.54 mm Hg)and Pa O2/Fi O2(196.63±55.54 mm Hg)were statistically significant(P<0.05).Blood pressure declined by 6.66%after treatment,with 0%skin mucosa injury and 8.33%stomach contents vomiting occurred.Conclusion For patients with ARDS,PPV is helpful for the improvement of arterial oxygenation,yet has no negative effects on hemodynamics and complications.Therefore,it is of significant value to be popularized in clinical application.
