摘 要:目的探讨补肺阳方改善慢性阻塞性肺疾病(COPD)免疫失衡的作用机制。方法60只Wistar大鼠随机分为空白对照组、模型组、补肺阳方高、中、低浓度组、胸腺肽组,每组10只。采用烟熏加气管滴注脂多糖及二次造模的方法建立大鼠COPD肺阳虚证模型,解剖胸腺、脾脏,计算胸腺指数、脾脏指数;流式细胞仪检测脾脏及血清T淋巴细胞CD4^+、CD8^+表达率,并计算CD4^+、CD8^+比值;采用酶联免疫法测定大鼠血清IgA、IgM、IgG水平。结果模型组大鼠支气管、肺组织形态符合COPD,补肺阳方可改善其形态结构;与空白组比较,模型组大鼠脾脏指数变化不明显(P>0.05),胸腺指数低于空白组(P<0.05),各治疗组与模型组比较差异无统计学意义(P<0.05);模型组大鼠脾脏CD4^+/CD8^+值低于空白对照组(P<0.05),除低剂量组外,其余治疗各组CD4^+/CD8^+值均高于模型组(P<0.05),外周血CD4^+/CD8^+比值变化各组间比较差异无统计学意义(P>0.05);模型组大鼠血清IgA、IgM、IgG水平均低于空白对照组(P<0.01),治疗各组IgA、IgM和IgG水平均较模型组升高(P<0.05,P<0.01)。结论补肺阳方能通过提高模型大鼠CD4^+/CD8^+比值、血清中IgA、IgM、IgG水平,调节机体体液免疫和细胞免疫,从而改善COPD免疫失衡状态。Objective To observe the effects of Bufeiyang Recipe(BR)on immunologic function of chronic ob-structive pulmonary disease(COPD)rats with Fei yang deficiency syndrome(LYDS).Methods Totally 60 Wistar rats were randomly divided into the blank control group,the model group,high,middle,low dose BR groups,and thymosin posi-tive control group,10 in each group.COPD rat with LYDS model was established by cigarette smoke exposure and intratra-cheal instillation of lipopolysaccharide(LPS).After the dissection of the thymus and spleen,thymus index and spleen index were calculated.Contents of CD4^+and CD8^+in spleen and serum were detected by flow cytometry,and CD4^+/CD8^+ratio was calculated.The serum levels of immunoglobulin IgA,IgG and IgM were determined by ELISA.Results The morphologies of bronchus and lung tissue were in line with those of COPD.BR ameliorated these morphological structures.Compared with the blank control group,no obvious changes were seen in spleen index of the model group(P>0.05).But thymus index was lower in the model group than in the blank control group(P<0.05).No statistical difference existed between each treatment group and the model group(P>0.05).The ratio of spleen CD4^+/CD8^+was significantly lower in model group than in the blank control group(P<0.05),and the ratio of CD4^+/CD8^+in all treatment groups except low dose BR group were significantly higher than the model group(P<0.05).No statistical difference in peripheral CD4^+/CD8^+existed among all treatment groups(P>0.05).Serum IgA,IgG,and IgM levels were lower in the model group than in the blank control group(P<0.01).Serum IgA,IgG,and IgM levels were lower in all treatment groups than in the model group(P<0.05,P<0.01).Conclusion BR ameliorated immune disturbance by elevating CD4^+/CD8^+and serum levels of IgA,IgG,and IgM,regulating humoral immunity and cell immunity.
