MSCT所测正常人肝脏体积与性别、年龄及相关因素关系分析

摘　　要：目的采用16层螺旋CT测量正常成人全肝及左右半肝体积,探讨正常人群肝脏体积与身高、体重、体表面积及体块指数之间的关系,进一步探讨影响测量准确性的相关因素。方法对200例正常成年人[男115例,女85例,年龄20~80岁,平均(53.23±14.49)岁]行上腹部CT增强扫描,采用测量软件对全肝及左右半肝体积进行定量测量,同时测量其身高、体重,并对不同性别及年龄分组进行比较。课题设计前采用5个猪肝标本进行CT体积测量的实验研究,校正CT测量结果。结果正常成年男性肝脏体积及左右半肝体积分别为(1295.91±303.35)cm3、(385.96±116.13)cm3、(909.95±243.88)cm3;女性分别为(1146.29±291.59)cm3、(353.82±107.00)cm3、(792.46±215.47)cm3,男女不同性别间全肝及左右半肝体积差异有统计学意义(P<0.05);正常成年人不同年龄组间(20~40岁、41~60岁、61~80岁)全肝体积及左右半肝体积差异有统计学意义(P<0.05),40~60岁肝脏体积达到最大。正常成年男性、女性与体重、体表面积及体块指数呈正相关,分别为:体重男(r=0.511 P=0.000)、女(r=0.368 P=0.001)、体表面积男(r=0.543 P=0.000)、女(r=0.406 P=0.000)、体块指数男(r=0.458 P=0.000)、女(r=0.320 P=0.003);年龄、身高对成人肝脏体积大小影响无显著相关性。结论MSCT可客观测量全肝体积及左右半肝体积值,男女不同性别间存在明显差别;正常人肝脏体积与体重、体表面积及体块指数具有肯定相关性。肝脏各叶体积正常参考值的提出将对肝脏移植等临床应用提供重要指导价值。Objective To measure the whole liver volume(LV)and left liver lobe volume(LLV)and right liver lobe volume(RLV)in normal adults by a 16-slice spiral CT,and to explore the relationship between liver volume and height,weight,body surface area and body mass index in the normal population,so as to explore the related factors affecting the accuracy of the measurement.Methods A total of 200 normal adults[115 males and 85 females,aged 20~80 years,with an average age of(53.23±14.49)years]were enrolled in the study,and CT scan of the upper abdomen was performed,and the measurement software was used to quantitatively measure the LV,LLV and RLV.In addition,their height and weight were measured,and they were divided into different groups according to different gender and age for comparison.Before the design of the project,five pig liver specimens were used for the experimental research on volume measurement by CT to correct the CT measurement results.Results For normal adult males,LV,LLV and RLV were(1,295.91±303.35)cm3,(385.96±116.13)cm3,and(909.95±243.88)cm3,respectively.For females,LV,LLV and RLV were(1,146.29±291.59)cm3,(353.82±107.00)cm3,and(792.46±215.47)cm3,respectively.There were significant differences in the levels of LV,L-LV and R-LVs between males and females(P<0.05).Moreover there were significant differences in LV,LLV and RLV among different age groups of adults(20~40 years group,40~60 years group,60~80 years group)(P<0.05),with the maximum volume of livers being in 40~60 years group.Normal adult males and females were positively correlated with body weight,body surface area and body mass index,respectively:weight of males(r=0.511,P=0.000)and of females(P<0.01),body surface area of males(P<0.01)and of females(P<0.01),body mass index of males(P<0.01)and of females(P<0.01).However,age and height were not correlated with the liver volume of adults.Conclusion MSCT can objectively measure LV,LLV and RLV,and there are significant differences between males and females.The liver volume of normal people has a posi
