摘 要:目的探讨前S1抗原(pre S1 antigen,Pre S1-Ag)、乙型肝炎病毒外膜大蛋白(hepatitis B virus large surface protein,HBV-LP)、5'-核苷酸酶(5'-nucleoticlase,5'-NT)与常规的乙肝5项检测(HBV markers,HBV-M)、病毒载量的相关性。方法选择185例乙肝患者作为观察组,同期体检的50例健康人群作为对照组,测定血清Pre S1-Ag、HBV-LP、5'-NT、HBV-M和HBV-DNA水平。分析Pre S1-Ag、HBV-LP、5'-NT与HBV-M、HBV-DNA的相关性。结果观察组血清Pre S1-Ag、HBV-LP和5'-NT高于对照组(t=26.713,P<0.01;t=48.669,P<0.01;t=15.067,P<0.01);185例患者共检出5种HBV-M模式,5种模式患者的Pre S1-Ag、HBV-LP和5'-NT表达差异有统计学意义(x^2=19.170,P=0.001;x^2=17.365,P=0.002;F=14.655,P=0.001),其中HBeAg阳性患者血清Pre S1-Ag、HBV-LP和5'-NT表达高于阴性患者(x^2=19.136,P<0.01;x2=17.105,P<0.01;F=5.882,P<0.01)。不同HBV-DNA载量(<500、500~<104、104~<107、≥107)患者血清Pre S1-Ag和HBV-LP差异有统计学意义(F=96.523,P<0.01;F=733.041,P<0.01;F=18.994,P<0.01),HBV-DNA与Pre S1-Ag、HBV-LP呈正相关(r=0.647,P<0.01;r=0.753,P<0.01)。结论Pre S1-Ag、HBV-LP与病毒复制HBV-DNA呈正相关,与HBV-M密切相关,可作为HBV-DNA检测的有效补充。Objective To explore the correlations between pre-S1 antigen(Pre S1-Ag),hepatitis B virus large protein(HBV-LP)and 5'-nucleotidase(5'-NT)with routine HBV markers(HBV-M)and viral load(HBV-DNA).Methods One hundred and eighty-five hepatitis B patients were selected as the observation group and 50 healthy subjects were selected as the control group.Serum Pre S1-Ag,HBV-LP,5'-NT,HBV-M and HBV-DNA were measured.The correlation between Pre S1-Ag,HBV-LP and 5'-NT with HBV-M,HBV-DNA was analyzed.Results The serum levels of Pre S1-Ag,HBV-LP and 5'-NT in the observation group were higher than those in the control group(t=26.713,P<0.01;t=48.669,P<0.01;t=15.067,P<0.01).A total of 5 HBV-M models were detected in 185 patients.The differences in expressions of Pre S1-Ag,HBV-LP and 5'-NT under the five models were statistically significant(x^2=19.170,P=0.001;x^2=17.365,P=0.002;F=14.655,P=0.001).The expression of serum Pre S1-Ag,HBV-LP and 5'-NT in HBeAg-positive patients were higher than those in negative patients(x^2=19.136,P<0.01;x^2=17.105,P<0.01;F=5.882,P<0.01).The differences of serum Pre S1-Ag,HBV-LP,and 5'-NT among patients with different HBV-DNA(<500,500 to<104,104 to<107 and≥107)were statistically significantly different(F=96.523,P<0.01;F=733.041,P<0.01;F=18.994,P<0.01).HBV-DNA was positively correlated with Pre S1-Ag,HBV-LP,and 5'-NT(r=0.647,P<0.01;r=0.753,P<0.01).Conclusion Pre S1-Ag,HBV-LP and 5'-NT showed positive correlation with HBV-M and HBV-DNA,and can be used as effective supplement for HBV-DNA detection.
