摘　　要：目的探究慢性乙型肝炎病毒(hepatitis B virus,HBV)感染患者外周血iNKT细胞因子、高尔基体蛋白73(GP73)、骨桥蛋白(osteopontin,OPN)表达意义及与感染状态的关系。方法选取我院2018年1月—2019年8月慢性HBV感染患者106例作为观察组,其中HBV携带者33例、慢性乙肝者47例、乙肝肝硬化者26例。检测对比2组外周血血清iNKT细胞因子(干扰素γ(IFN-γ)、肿瘤坏死因子-α(TNF-α))、GP73、OPN水平。比较不同血清水平患者HBV-DNA表达量,Pearson分析各指标与HBV-DNA表达量关联性。同时检测102例健康体检者血清IFN-γ、TNF-α、GP73、OPN水平作为对照。结果观察组血清IFN-γ、TNF-α、GP73、OPN水平与对照组相比,差异均有统计学意义(F=217.438,P<0.01;F=140.290,P<0.01;F=851.922,P<0.01;F=443.249,P<0.01);血清IFN-γ、TNF-α、GP73、OPN与HBV-DNA表达量呈正相关(r=0.445,P<0.01;r=0.317,P<0.01;r=0.403,P<0.01;r=0.369,P<0.01)。结论血清iNKT细胞因子、GP73、OPN与HBV-DNA表达量存在正相关关系,动态监测上述血清指标水平,可为临床评估慢性HBV感染状态、判断预后提供循证指导。Objective To investigate the significance of iNKT cytokines,Golgi protein 73(GP73)and osteopontin(OPN)expressions in peripheral blood of patients with chronic hepatitis B virus(HBV)infection and the relationship of the expressions with infection status.Methods From January 2018 to August 2019,106 patients with chronic HBV infection in our hospital were selected as the observation group,including 33 cases of HBV carriers,47 cases of chronic hepatitis B,and 26 cases of hepatitis B cirrhosis.The levels of serum iNKT cytokines[interferon gamma(IFN-γ),tumor necrosis factor-α(TNF-α)],GP73 and OPN in peripheral blood of the two groups were detected.HBV-DNA expression levels of patients with different serum levels of above indexes were compared.Pearson analysis was used for the correlation between each index and HBV-DNA expression.At the same time,the serum IFN-γ,TNF-α,GP73,OPN levels of 102 healthy people were detected as controls.Results The serum IFN-γ,TNF-α,GP73,OPN levels in the observation group were significantly different from those in the control group(F=217.438,P<0.01;F=140.290,P<0.01;F=851.922,P<0.01;F=443.249,P<0.01,respectively).The serum IFN-γ,TNF-α,GP73,OPN were positively correlated with HBV-DNA expression levels(r=0.445,P<0.01;r=0.317,P<0.01;r=0.403,P<0.01;r=0.369,P<0.01,respectively).Conclusions The positive correlation between serum iNKT cytokines,GP73 and OPN with HBV-DNA expression existed.Dynamic monitoring of these serum indicators can provide evidence-based guidance for clinical assessment of chronic HBV infection status and judgment of prognosis.
