腹腔镜下广泛性子宫切除联合淋巴结清扫术治疗宫颈癌的效果及对患者生活质量的影响

摘　　要：目的探讨腹腔镜下广泛性子宫切除联合淋巴结清扫术治疗宫颈癌的效果及对患者生活质量的影响。方法选取60例宫颈癌手术患者,通过计算机抽选随机数字,将研究对象分为研究组(n=30)和对照组(n=30)。对照组患者采取开腹子宫切除术,研究组患者采取腹腔镜下广泛性子宫切除联合淋巴结清扫术,对比2种手术方案对宫颈癌患者手术效果及生活质量的影响。结果研究组患者手术时间长于对照组(P<0.05);研究组患者肛门排气时间、导尿管留置时间、平均卧床时间、住院时间明显短于对照组(P<0.05),研究组患者手术出血量明显少于对照组(P<0.05)。研究组并发症发生率为13.3%(4/30),低于对照组的36.7%(11/30),组间比较差异显著(P<0.05)。术后6个月随访,研究组患者的生活质量评分高于对照组(P<0.05)。结论腹腔镜下广泛性子宫切除联合淋巴结清扫术治疗宫颈癌,具有良好的手术效果,并发症发生率低,安全性高,可减少患者机体创伤程度,保证治疗效果,改善预后,提高患者的生活质量,值得临床上推广应用。Objective To investigate the effect of laparoscopic extensive hysterectomy combined with lymph node dissection on the surgical outcome and quality of life of patients with cervical cancer.Methods 60 patients with cervical cancer were randomly divided into the study group(n=30)and the control group(n=30)by computer randomization.The control group underwent open hysterectomy,the study group underwent laparoscopic extensive hysterectomy combined with lymphadenectomy,comparing the effect of the 2 surgical options on the surgical outcome and quality of life of cervical cancer patients.Results The operation time of the study group was significantly longer than that of the control group(P<0.05).The anus exhaust time,catheter indwelling time,average bed rest time,and hospitalization time were significantly shorter in the study group than in the control group(P<0.05).The amount of surgical bleeding was significantly lower in the patients than in the control group(P<0.05).The incidence of complications in the study group was 13.3%(4/30),which was lower than 36.7%(11/30)in the control group,with significant difference between the 2 groups(P<0.05).Follow-up at 6 months after surgery,the quality of life scores of the study group were significantly higher than those of the control group(P<0.05).Conclusion Laparoscopic hysterectomy combined with lymphadenectomy for cervical cancer has good surgical results,low complication rate and high safety.It can reduce the degree of trauma of the patient,ensure the therapeutic effect,improve prognosis and quality of life,is worthy of clinical application.
