摘  要:目的基于免疫组织化学技术观察秦艽不同配伍对风寒湿痹类风湿关节炎(RA)大鼠踝关节核因子κB(NF-κB)、血管内皮生长因子(VEGF)表达的影响并观察大鼠踝关节病理学改变。方法80只实验大鼠随机分为空白组、Ⅱ型胶原模型组、风寒湿病证模型组(Ⅱ型胶原+风寒湿刺激)、阳性药组、单味秦艽组、秦威组(秦艽-威灵仙)、秦桑组(秦艽-桑寄生)、秦防组(秦艽-防己),每组10只。采用注射Ⅱ型胶原乳剂诱导并给予风寒湿刺激制备风寒湿RA大鼠模型。模型建立完成后各给药组给予相应药液灌胃,其余组给予生理盐水。采用免疫组织化学技术检测踝关节NF-κB、VEGF的表达;苏木精(Hatmatoxylin)-伊红(Eosin)染色(HE染色)观察踝关节病理学改变。结果与空白组比较,Ⅱ型胶原组及病证模型组踝关节NF-κB、VEGF蛋白表达水平显著升高(P<0.01);与风寒湿病证模型组比较,各给药组踝关节NF-κB、VEGF表达水平均降低,中药组中以秦威组降低最明显(P<0.01)。HE染色结果显示,造模后Ⅱ型胶原组软骨组织结构严重破坏,软骨细胞排列疏松,少量软骨细胞核固缩凋亡,大量炎细胞浸润,大量新生血管生成,关节面严重破损;病证模型组在Ⅱ型胶原组病理改变基础上关节腔内存在大量纤维物渗出,滑膜细胞增生,部分滑膜细胞呈绒毛状增生突入关节腔内,关节表面粗糙;经治疗后,各治疗组大鼠踝关节软骨组织结构破坏减少,少量炎细胞浸润,新生血管生成减少,关节面破损有不同程度恢复。结论平温相配的秦威组对于风寒湿痹型类风湿关节炎治疗效果优于平平相配的秦桑组、平寒相配的秦防组及单味秦艽组,实验结果与中医"寒者热之"的治疗原则基本相符。该配伍治疗类风湿关节炎的机制可能与其可以降低NF-κB、VEGF的表达,减轻关节破坏,减少新生血管有关。Objective The effects of different compatibility of Qinjiao(Gentianae Macrophyllae Radix)on the expressions of nuclear factorκB(NF-κB)and vascular endothelial growth factor(VEGF)in the ankle of rheumatoid arthritis(RA)model rats were observed by immunohistochemical techniqueand the pathological changes of the ankle were observed to explore the mechanism of different compatibility of Qinjiao(Gentianae Macrophyllae Radix)on the treatment of rheumatoid arthritis with wind-cold-dampness arthralgia.Methods Eighty experimental rats were randomly divided into blank group,collageⅡmodel group,wind-cold-dampness syndrome model group(typeⅡcollage+wind-cold-dampness stimulation),positive drug group,single Qinjiao(Gentianae Macrophyllae Radix)group,Qinwei group,Qinsang group[Qinjiao(Gentianae Macrophyllae Radix)-Sangjisheng(Taxillus sutchuenensis)],Qinfang group[Qinjiao(Gentianae Macrophyllae Radix)-Fangji(Stephania tetrandra)],10 rats in each group.Rat model of wind-cold-dampness RA was induced by injection of collagenⅡemulsion and stimulated by wind-cold-dampness.After the establishment of the model,each group was given the corresponding medicine and the other groups were given normal saline.Expressions of NF-κB and VEGF in ankle joint were detected by immunohistochemistry.Hematoxylineosin staining(HE staining)was used to observe the pathological changes of ankle joint.Results Compared with the blank group,expressions of NF-κB and VEGF protein in ankle joint of collageⅡmodel group and wind-cold-dampness syndrome model group were significantly increased(P<0.01).Compared with the wind-cold-dampness syndrome model group,the expressions of NF-κB and VEGF in ankle joint of each administration group decreased,especially in Qinwei[Qinjiao(Gentianae Macrophyllae Radix)-Weilingxian(Clematis chinensis Osbeck)]group(P<0.01).HE staining results showed that the cartilage tissue structure of collageⅡmodel group was seriously damaged.The chondrocytes were loosely arranged.A small number of chondrocytes were condensed and ap
