摘　　要：目的:探讨1.5T 磁共振成像技术(Magnetic resonance imaging,MRI)在子宫剖宫产瘢痕妊娠(Cesarean scar pregnancy,CSP)诊断应用价值,以提高对其诊断的准确率。方法:选择性从2017年3月至2019年3月收治30例患者经临床手术或病理证实为子宫剖宫产瘢痕妊娠患者,临床资料完整,术前均行MRI检查(平扫21例,平扫+增强扫描9例),与超声诊断结果相比较。结果:MRI检查正确诊断剖宫产瘢痕妊娠30例,诊断准确率100%。超声检查正确诊断剖宫产瘢痕妊娠26例,诊断准确率86.7%,二种检查方法在剖宫产瘢痕妊娠诊断准确率方面MRI优于超声检查,经确诊病例临床采取积极治疗均获得满意效果。结论:子宫剖宫产瘢痕妊娠在MRI方面具有一定的特异性,既可作为诊断CSP的首选确诊检查方法,又可作为超声检查方法的重要补充应用于临床,如二者联合应用为剖宫产瘢痕妊娠诊断及进一步优化临床治疗提供重要的参考价值。Objective To investigate the application value of 1.5T magnetic resonance imaging(MRI)in the diagnosis of cesarean scar pregnancy(CSP),in order to improve the accuracy of diagnostic.Methods Selective from March 2017 to March 2019 were 30 cases of patients confirmed by clinical surgery or pathological confirmation were CSP,complete clinical data,MRI in preoperative(21 patients with plain scan and 9 patients with plain scan+enhanced scan),compared with the results of ultrasonic diagnosis.Results MRI examination correctly diagnosed CSP in 30 cases,and the diagnostic accuracy was 100%.Ultrasonic examination correctly diagnosed CSP in 26 cases,with a diagnostic accuracy of 86.7%.Two kinds of examination methods MRI were superior to ultrasonic examination in the diagnosis accuracy rate of CSP,and satisfactory results were obtained after positive clinical treatment of confirmed cases.Conclusion CSP has a certain specificity in MRI,which can be used as the first choice for diagnosis of CSP and as an important supplement of ultrasonic examination.The combined application of CSP the two examination methods can provide important reference value for diagnosis of CSP and further optimization of clinical treatment.
