化止利膏治疗肝肾阴虚型IgA肾病血尿的临床研究

摘　　要：目的观察化止利膏治疗肝肾阴虚型IgA肾病血尿患者的疗效及安全性,为临床防治该病提供新的思路和方法。方法收集2017年4月—2018年10月在上海市杨浦区中医医院经肾穿刺活检确诊且辨证属肝肾阴虚型IgA肾病血尿患者60例,按随机数字表法随机分为治疗组和对照组,每组30例。治疗组在西医常规治疗基础上予以化止利膏治疗,对照组在西医常规治疗基础上予以血尿安胶囊治疗,2组均治疗24周。观察2组患者治疗前后的尿红细胞计数、血红蛋白、中医症状积分,记录2组患者临床疗效及不良反应发生情况。结果2组治疗后尿液镜下红细胞数较治疗前明显减少(P均<0.05),中医症状积分较治疗前明显降低(P均<0.05),且治疗组均明显少于或低于对照组(P均<0.05);治疗组治疗后血红蛋白水平明显高于治疗前(P<0.05),但与对照组治疗后比较差异无统计学意义(P>0.05);治疗组与对照组总有效率分别为90.0%(27/30),83.3%(25/30),治疗组明显高于对照组(P<0.05)。治疗期间,2组均无明显不良反应,2组治疗后肝肾功能等指标(尿素氮、血肌酐、总胆红素、谷丙转氨酶、谷草转氨酶、白蛋白、血钾)均无异常(P均>0.05)。结论化止利膏治疗肝肾阴虚型IgA肾病血尿疗效明显优于血尿安胶囊,不仅可明显减少患者的尿红细胞数,提高血红蛋白水平,改善患者的中医症状,且安全。Objective It is to observe the efficacy and safety of Huazhili ointment in the treatment of IgA nephropathy hematuria with Yin deficiency type of liver and kidney,and provide new ideas and methods for the clinical prevention and treatment of the disease.Methods Sixty cases of patients with IgA nephropathy hematuria with Yin deficiency type of liver and kidney,who visited Yangpu District Hospital of Traditional Chinese Medicine from April 2107 to October 2018,confirmed by renal puncture biopsy were randomly divided into the treatment group and the control group,30 cases in each group.Based on the routine western medicine treatment,the treatment group was treated with Huazhili ointment,the control group was treated with Xueniaoan capsule.Both groups were treated for 24 weeks.The urine red blood cell counts,hemoglobin,scores of traditional Chinese medicine symptoms before and after treatment were observed between the two groups,and adverse reactions happened were recorded.Results Compared with before treatment,the urine red blood cell counts in both group were decreased obviously(P both<0.05),scores of traditional Chinese medicine symptoms were also declined remarkably(P both<0.05).Compared with the control group,the urine red blood cell counts and scores of traditional Chinese medicine symptoms in the treatment group were reduced significantly(P both<0.05).Hemoglobin was increased significantly after treatment in the treatment group,but there was no significant difference between the treatment group and the control group(P>0.05).The total effective rates in the treatment group and control group were 90.0%(27/30),83.3%(25/30),respectively.The total effective rates in the treatment group was obviously higher than that in the control group(P<0.05).During the treatment period,there was no obvious adverse reactions were reported in the two groups,and the indicators of liver and kidney function(urea nitrogen,blood creatinine,total bilirubin,alanine aminotransferase,aspartate aminotransferase,albumin,serum potassium)in bo
