摘 要:目的分析新生儿急腹症的X线影像学表现,探讨其在不同急腹症中的诊断价值,为改善实验室诊断效果提供依据。方法回顾性分析我院212例经临床及手术病因确诊的新生儿急腹症X线影像学表现,比较其在各类新生儿急腹症中的准确率,分析其未能得出正确诊断结果的原因,总结其X线的特征。结果212例中,X线诊断的准确率在胃肠穿孔16例、肛门闭锁41例、膈疝5例达到100%;在食管闭锁94%(16/17)和坏死性小肠结肠炎92%(55/61)等疾病中亦有良好表现;而在其他类型准确率低于90%,分别为:先天性脐膨出及腹裂86%(6/7)、肠闭锁85%(12/14)、先天性巨结肠84%(21/25)、肠旋转不良77%(14/18)、胎粪性腹膜炎75%(6/8)。结论X线影像诊断在不同新生儿急腹症中具有不同的准确率,在未发现高准确率急腹症影像学特征的病例,应考虑结合其他检查手段,如超声、CT及MRI以提高诊断准确率。Objective To analyze the X-ray imaging manifestations of neonatal acute abdomen disease,to explore its diagnostic value in different acute abdominal diseases,and to provide a basis for improving the laboratory diagnosis.Methods A total of 212 cases of neonatal acute abdomen diagnosed by clinical and surgical etiology in X-ray imaging in our hospital were retrospectively analyzed.Their accuracy rates in different types of neonatal acute abdomen were compared to analyze the cause for the misdiagnosis and summarize their X-ray characteristics.Results In 212 cases,the accuracy of X-ray diagnosis reached 100%in gastrointestinal perforation(n=16),anal atresia(n=41),and diaphragmatic hernia(n=5);And it also performed well in esophageal atresia[94%(16/17)]and necrotizing enterocolitis[92%(55/61)];While the accuracy rate was less than 90%in other types,namely:congenital umbilical bulge and abdominal fissure[86%(6/7)],intestinal atresia[85%(12/14)],congenital megacolon[84%(21/25)],intestinal malrotation[77%(14/18)],and meconium peritonitis[75%(6/8)].Conclusion X-ray imaging diagnosis displays different accuracy rates in different neonatal acute abdominal diseases.In cases where high-accuracy imaging features of acute abdomen are not found,other examination methods such as ultrasound,CT and MRI should be considered to enhance the diagnostic accuracy.
