我院500例艾滋病患者高效联合抗逆转录病毒治疗用药分析

摘　　要：目的促进临床合理选择高效联合抗逆转录病毒治疗(HAART)方案治疗艾滋病。方法随机抽取2019年3月至5月到医院药房取药的500例艾滋病患者,与其直接沟通,调查内容包括患者的年龄、曾服用和正服用的药品、服药时间、换药原因、对现服药品是否满意、是否有漏服情况等。结果替诺福韦(TDF)+拉米夫定(3TC)+依非韦伦(EFV),TDF+3TC+洛匹那韦/利托那韦[LPV(r)],齐多拉米双夫定[齐多夫定(AZT)+3TC]+EFV是使用人数排前3位的HAART方案,药品的用法用量直接影响患者的漏服率和依从性,利匹韦林(RPV)是EFV和LPV(r)最常见的替代药物,20~59岁男性患者是目前当地最多见的患病人群。治疗的第7至9年中,只有服用TDF+3TC+EFV组合的患者可以继续服用。结论HAART能有效抑制艾滋病患者体内人类免疫缺陷病毒(HIV)复制,控制机会性感染和减少病毒传播,改善患者的生命质量,对于降低艾滋病发病率和病死率具有决定性意义。Objective To promote the rational clinical selection of highly active antiretroviral therapy(HAART)for AIDS.Methods A total of 500 AIDS patients who got the medicine at the pharmacy of our hospital from March to May 2019 were randomly selected for face-to-face communication,including:the age of the patient,the drugs taken previously and at present,the medication time,the cause for changing drugs,satisfaction with the drugs administered currently and whether there was any missedadministration of drugs.Results TDF+3 TC+EFV,TDF+3 TC+LPV(r)and(AZT+3 TC)+EFV were the top three HAART drugs.The usage and dosage of drugs had direct impact on the missed administration rate and compliance of the patient.RPV was an alternative medicine of EFV and LPV(r).Male patients aged 20-59 years were the most common patient group in the region.During the 7 th to 9 th year of treatment,only patients taking the combination of TDF+3 TC+EFV could continue to take it for 7-9 years.Conclusion HAART can effectively inhibit the replication of human immunodeficiency virus HIV in AIDS patients,control opportunistic infections and reduce the spread of the virus,improve the quality of life of patients,and have a decisive significance for reducing the incidence and mortality of AIDS.
