摘　　要：目的探讨三七接骨丸联合有限内固定、微型外固定架治疗手部骨折的临床疗效。方法选取新乐市医院2017年1月至2018年3月收治拟行手术治疗的手部骨折患者46例,按入院先后顺序单、双编号,分为观察组和对照组,各23例。两组患者均予微型外固定架结合克氏针有限内固定术,并予常规治疗药物干预,以及口服碳酸钙D3片;观察组患者增服三七接骨丸。均连续治疗2个月。结果观察组患者手部功能优良率为95. 65%,显著高于对照组的73. 91%(P <0. 05);观察组患者骨折愈合时间、视觉模拟评分(VAS)均显著低于对照组,生活质量评分显著高于对照组(P <0. 05);与治疗前比较,两组患者治疗后的血清骨钙素(BGP)、护骨因子(OPG)、胰岛素样生长因子1(IGF-1)、骨形成蛋白(BMP)水平均显著升高,观察组患者上述指标改善程度均显著优于对照组;观察组与对照组不良反应发生率相当(17. 39%比13. 04%,χ~2=0. 169,P=0. 681> 0. 05)。结论三七接骨丸联合有限内固定、微型外固定架治疗手部骨折,能加速骨折愈合,减轻疼痛感,提高生活质量,改善手部活动功能,并提升各项骨形成标志物指标水平。Objective To discuss the clinical efficacy of Sanqi Jiegu Pills combined with limited internal fixation and miniature external fixator on hand fracture. Methods A total of 46 patients with hand fracture who underwent surgical treatment in Xinle City Hospital from January 2017 to March 2018 were selected. They were numbered according to the sequence of hospitalization and randomly divided into the observation group and control group,23 cases in each group. Both groups were treated with miniature external fixator combined with limited internal fixation with Kirschner wire. The patients in the control group were treated with routine drugs and calcium carbonate D3 tabletsorally after operation;the patients in the observation group were treated with Sanqi Jiegu Pills orally on the basis of the control group. Both groups were continuously treated for 2 months. Results The excellent and good rate of hand function in the observation group was 95. 65%,which was significantly higher than73. 91% in the control group( P < 0. 05). The fracture healing time and visual analogue score( VAS) of the observation group were significantlylower than those of the control group,and the quality of life score was significantlyhigher than that of the control group( P < 0. 05). Compared with before treatment,the serum osteocalcin( BGP),osteoprotegerin( OPG),insulin-like growth factor-1( IGF-1) and bone morphogenetic protein( BMP) in the two groups were higher than those before treatment,and the improvements in the observation group were significantly better than those in the control group( P < 0. 05). The incidence of adverse reactions in the observation group and the control group was equivalent( 17. 39% vs. 13. 04%,χ~2= 0. 169,P = 0. 681 > 0. 05). Conclusion Sanqi Jiegu Pills combined with limited internal fixation and mini external fixator on hand fracture can accelerate the healing of fracture,reduce pain,improve the quality of life,improve the function of hand movement,and improve the level of bone formation markers.
