摘 要:目的探讨布地奈德联合阿奇霉素序贯治疗小儿迁延性支原体肺炎的临床疗效及对患儿炎性因子水平和免疫功能的影响。方法选取医院2017年1月至2019年1月收治的迁延性支原体肺炎患儿98例,随机分为观察组和对照组,各49例。两组患儿均予退热、止咳、祛痰、平喘、氧疗等基础治疗及阿奇霉素序贯治疗,观察组患儿加用布地奈德吸入气雾剂治疗。结果观察组总有效率为89.80%,明显高于对照组的73.47%(P<0.05);观察组患儿体温降至正常、喘息消失、咳嗽消失、肺部湿罗音消失的时间均明显短于对照组(P<0.05);治疗后,两组患儿C反应蛋白(CRP)、红细胞沉降率(ESR)、白细胞介素6(IL-6)水平均明显降低,免疫球蛋白A(IgA)、免疫球蛋白G(IgG)、免疫球蛋白M(IgM)水平均明显升高,且观察组均明显优于对照组(P<0.05);观察组不良反应发生率为18.37%,与对照组的10.20%相当(P>0.05)。结论布地奈德联合阿奇霉素序贯治疗小儿迁延性支原体肺炎,能快速改善临床症状,降低炎性因子水平,改善免疫功能。Objective To study the efficacy of azithromycin combined with budesonidesequential therapy on persistent mycoplasma pneumonia and the effects on inflammatory level and immune function in children.Methods A total of 98 children with persistent mycoplasma pneumonia who were treated in the hospital from January 2017 to January 2019 were selected and divided into the observation group and control group according the random number table method,49 cases in each group.The control group was treated with azithromycin alone,while the observation group was treated with azithromycin combined with budesonide inhaled aerosolsequentially.Results The total effective rate of the observation group was 89.80%,which was significantly higher than 73.47%of the control group(P<0.05).The time of temperature dropping to normal,wheezing disappearing,cough disappearing and lung moist rale disappearing in the observation group were significantly lower than those in the control group(P<0.05).After treatment,the values of C-reactive protein(CRP),erythrocyte sedimentation rate(ESR),interleukin 6(IL-6)and other inflammatory indicators in the two groups were significantly decreased,and the levels of immunoglobulin A(IgA),IgG and IgM were significantly increased,and the improvements in the observation group were significantly better than those of the control group(P<0.05).The incidence of adverse reactions was 18.37%in the observation group,which was equivalent to 10.20%in the control group(P>0.05).Conclusion Azithromycin combined with budesonide sequential therapy for persistent mycoplasma pneumonia in children can quickly improve the clinical symptoms,reduce the level of inflammatory factors,and improve the immune function of children.
