冠心病患者ApoA-1、HDL水平的变化及其临床意义

摘　　要：目的分析冠心病(CHD)患者血浆中载脂蛋白A-1(ApoA-1)、血清中高密度脂蛋白(HDL)水平的变化及其临床意义。方法选取2017年2月至2018年9月我院收治的102例CHD患者为CHD组,根据病情的严重程度分为轻度CHD组、中度CHD组及重度CHD组,各35、35、32例,另选取同期体检健康者48例作为对照组。比较各组受试者ApoA-1、HDL水平,并分析其在CHD患者病情评估中的作用。结果ApoA-1、HDL水平:对照组>轻度CHD组>中度CHD组>重度CHD组,差异具有统计学意义(P<0.05)。ApoA-1预测CHD的阳性预测值、阴性预测值、灵敏度、特异度分别为93.41%、71.19%、83.33%、87.50%,HDL的为85.37%、52.94%、68.63%、75.00%。结论ApoA-1、HDL作为CHD患者的预测指标,其水平变化对CHD患者会产生重要的影响,并且ApoA-1水平的变化对于CHD患者病情诊断的灵敏度更高。Objective To analyze the changes of plasma apolipoprotein A-1(ApoA-1)and serum high-density lipoprotein(HDL)levels in patients with coronary heart disease(CHD)and their clinical significance.Methods A total of 102 patients with CHD admitted in our hospital from February 2017 to September 2018 were selected as CHD group and they were divided into mild CHD group,moderate CHD group and severe CHD group according to the severity of the disease,with 35,35 and 32 cases in each group respectively.In addition,48 healthy persons with physical examination during the same period were selected as control group.The ApoA-1 and HDL levels of subjects in each group were compared,and their roles in the evaluation of CHD patients were analyzed.Results ApoA-1,HDL levels:the control group>the mild CHD group>the moderate CHD group>the severe CHD group,and the differences were statistically significant(P<0.05).The positive predictive value,negative predictive value,sensitivity and specificity of ApoA-1 to predict CHD were 93.41%,71.19%,83.33%,87.50%,and those in HDL were 85.37%,52.94%,68.63%,75.00%.Conclusion ApoA-1 and HDL are used as predictive indicators for CHD patients.The changes in their levels will have an important impact on patients with CHD,and changes in ApoA-1 levels are more sensitive to the diagnosis of the condition of CHD patients.
