社会因素剖宫产对产妇产后出血及母婴结局的影响

摘　　要：目的分析社会因素剖宫产(CDMR)对产妇产后出血及母婴结局的影响。方法选取2016年5月至2019年7月我院收治的110例分娩产妇进行回顾性分析,将57例自然分娩产妇设为对照组,53例CDMR产妇设为观察组。比较两组产妇产后2 h出血量、产后出血发生率、产妇应激状态及母婴结局。结果观察组产后2 h出血量、产后5、42 d子宫高度、盆腔黏连发生率及新生儿黄疸发生率均高于对照组(P<0.05)。产后24 h,两组ALD、NE水平均升高,且观察组高于对照组(P<0.05)。结论CDMR会增加产妇产后2 h出血量和新生儿黄疸发生率,使产妇处于较为强烈的应激状态,影响产妇和新生儿健康,在实际临床应用中,应给予产妇正确的分娩方式指导,以降低CDMR的发生率。Objective To analyze the influences of cesarean delivery on maternal request(CDMR)on postpartum hemorrhage of parturient and the outcomes of maternal and infant.Methods One hundred and ten parturients admitted in our hospital from May 2016 to July 2019 were selected for retrospective analysis.Fifty-seven cases of natural childbirth were set as control group,and fifty-three cases of CDMR were set as observation group.The amount of 2 h postpartum hemorrhage,the incidence of postpartum hemorrhage,the stress state of parturient and the outcomes of maternal and infant were compared between the two groups.Results The amount of 2 h postpartum hemorrhage,the height of uterus at 5 and 42 days after delivery,the incidences of pelvic adhesion and neonatal jaundice in the observation group were higher than those in the control group(P<0.05).At 24 h after delivery,the levels of ALD and NE in the two groups increased,and those in the observation group were higher than the control group(P<0.05).Conclusion CDMR can increase the amount of 2 h postpartum hemorrhage of parturient and the incidence of neonatal jaundice,which makes the parturient in a more intense state of stress and affects the health of the parturient and the newborn.In practical clinical application,we should give parturient the correct guidance of delivery mode to reduce the incidence of CDMR.
