针药配合振动疗法治疗徐动型脑瘫康复的应用

摘　　要：目的探讨针药配合振动系统治疗小儿脑瘫对神经、肌肉及脑发育的改善情况,运用Alberta评价量表与Gesell评价量表对各组脑瘫患儿在治疗前后的临床显效率进行评估;使用肌肉超声测定患儿双下肢腓肠肌内侧头横截面积,以此评价脑瘫患儿肌肉发育概况。方法将2017年3月-2018年11月收入沈阳市儿童医院康复科住院的94例脑瘫足月儿。其中男孩52名,女孩42名。按照数字表法随机将入院的脑瘫患儿进行随机分为两组:观察组与对照组各47例。治疗前及治疗2个疗程后运用Alberta运动量表与Gesell全身发育量表对脑瘫患儿神经行为学、运动行为学采取评价;使用肌肉超声检测三组患儿双下肢腓肠肌内侧头横截面积。结果两组患儿在治疗前使用Albert运动量表进行评估(P> 0. 05)无明显差异;经过2个疗程的治疗后,两组Albert运动量表进行评估均显著提高(P <0. 05)具有一定统计学意义,其中以针药配合振动组升高幅度最大,明显高、优于其对照组P <0. 05差异具有统计学意义;两组患儿在治疗前使用Gesell发育量表进行评估(P> 0. 05);经过2个疗程的治疗后,两组Gesell发育量表进行评估均显著提高(P <0. 05),但唯独针药配合振动组数据高幅度最大,对照组数据其次,与治疗前进行对比(P <0. 05);经过2个疗程的治疗后,与对照组对比,对照组的Gesell分值基本未升高(P <0. 05);在治疗后针药配合振动组左、右双下肢腓肠肌内侧头的横截面积均优于对照组(P <0. 05)。结论针药配合振动疗法治疗徐动型脑康复效果满意,值得推广。Objective To evaluate the clinical effect of children with cerebral palsy before and after treatment by Alberta scale and Gesell scale. To evaluate the muscle development of children with cerebral palsy by measuring the cross-sectional area of medial head of gastrocnemius muscle of lower limbs with muscle ultrasound. To explore the effect of systematic treatment of acupuncture and medicine combined with vibration on the neurological,muscular and cerebral development of children with cerebral palsy. Methods Ninety-four full-term infants with cerebral palsy were admitted to the rehabilitation department of Shenyang Children’s Hospital from March 2017 to November 2018. Among them,52 were boys and 42 were girls. Children with cerebral palsy admitted to hospital were randomly divided intoobservation group and control group with 47 cases in each.Neurobehaviology and motor behaviour of children with cerebral palsy were evaluated by Alberta motor scale and Gesell whole body development scale before and after treatment. The cross-sectional area of medial head of gastrocnemius muscle of lower limbs was measured by muscle ultrasound. Results There was no significant difference between the two groups in using Albert Motion Scale before treatment (P > 0. 05). After two courses of treatment,the Albert Motion Scale in both groups was significantly improved with a certain statistical significance (P < 0. 05),in which the increase in the acupuncture and medicine combined with vibration group was the largest,significantly higher than that in the control group (P < 0. 05). There was no significant difference in using Gesell Development Scale before treatment (P > 0. 05). After two courses of treatment,the evaluation of Gesell Development Scale in both groups was significantly improved (P < 0. 05),but the data of acupuncture and medicine combined with vibration group was the highest,and the data of control group was the second.
