天智颗粒对阴虚阳亢型缺血性中风神经功能缺损、肢体运动功能的影响

摘　　要：目的观察天智颗粒对阴虚阳亢型缺血性中风患者神经功能缺损及肢体运动功能的影响。方法选取缺血性中风患者98例,随机分为对照组和观察组。对照组给予基础治疗,观察组在对照组治疗的基础上加以天智颗粒,两组均连续治疗4周。测定治疗前后两组患者临床疗效、脑血流灌注状况、生活质量、神经缺损程度、血液流变学指标、中医证候临床评分、肢体运动功能、神经损伤指标、氧化应激指标及炎性因子。结果观察组的临床总有效率显著高于对照组(P<0.05)。治疗后观察组NIHSS评分、FMA评分及ADL评分均较优于对照组(P<0.05)。观察组患者肢体麻木、言语謇涩、眩晕耳鸣、心烦易怒、手足心热、咽干口燥各项积分均显著低于对照组(P<0.05)。治疗后两组MCA、ACA及PCA平均流速均明显升高(P<0.05),FIB、LBV及PV水平均显著降低(P<0.05),并且观察组的MCA、ACA、PCA、FIB、LBV及PV较对照组改善程度明显(P<0.05)。观察组治疗后NSE、S100B、GFAP、MDA及SOD水平均显著低于对照组(P<0.05);而两组TNF-α、IL-1β水平治疗后没有显著差异(P>0.05)。结论天智颗粒可显著改善阴虚阳亢型缺血性中风患者的神经缺损程度,并有效调节患者的肢体运动功能,促进脑部血液循环,值得临床推广使用。Objective To observe the effect of Tianzhi Granule on neurological deficit and limb motor function in patients with ischemic stroke of Yin deficiency and Yang hyperactivity type. Methods Ninety-eight patients with ischemic stroke were randomly divided into control group and observation group. The control group was given basic treatment, while the observation group was given Tianzhi Granule on the basis of the control group. Both groups were treated continuously for 4 weeks. The clinical efficacy, cerebral blood flow perfusion, quality of life, degree of nerve defect, hemorheological index, clinical score of TCM syndromes, limb motor function, nerve injury index, oxidative stress index and inflammatory factors were measured before and after treatment. Results The total clinical effective rate of the observation group was significantly higher than that of the control group(P<0.05). After treatment, the NIHSS score, FMA score and ADL score of the observation group were better than those of the control group(P<0.05). The scores of numbness of limbs, astringency of speech, vertigo and tinnitus, irritability, heat of hands and feet, dry throat and mouth in the observation group were significantly lower than those in the control group(P<0.05). After treatment, the mean flow velocity of MCA, ACA and PCA increased significantly in both groups(P<0.05).The levels of FIB, LBV and PV decreased significantly(P<0.05), and the MCA, ACA, PCA, FIB, LBV and PV in the observation group improved significantly compared with those in the control group(P<0.05). After treatment, the levels of NSE, S100 B, GFAP, MDA and SOD in the observation group were significantly lower than those in the control group(P<0.05), while there was no significant difference in the levels of TNF-alpha or IL-1 beta between the two groups(P>0.05).
