摘 要:目的了解和评价县级医院临床输血管理现状。方法建立"临床输血质控监查体系",分组方式以"体系"建立前后2017-2019年间同期时间段患者入组:分析整理2018年12月-2019年5月景谷县人民医院收治的需进行输血治疗的185例患者作为研究组,2017年12月-2018年5月县人民医院收治的需进行输血治疗的158例患者作为对照组。采用回顾性分析方法,以输血全过程系统的质量监控安全输血合格率、备血标本采集合格率、输血申请单送检合格率为指标,两组间数据的采用统计学t检验、差异性比较。结果输血全过程系统的质量监控安全合格率比较:研究组与对照组数据分别显示,输血前指征符合率为178例(96.22%)、127例(80.38%),输血治疗知情同意书签署的完整性为175例(94.59%)、130例(82.28%);输血中过程记录的完整性为179例(96.76%)、144例(91.14%);输血后输血后效果的评价为165例(89.19%)、127例(80.38%),2组数据比较t=5.518,P<0.01;输血备血标本采集、申请单送检合格率比较:研究组与对照组数据分别显示,备血标本采集为178例(96.22%)、143例(90.51%),申请单送检合格率为176例(95.14%)、139例(87.97%),2组数据比较t=8.822,P<0.01。结论严格输血管理,建立完善临床输血质控监查系统,对临床输血的每个环节进行合理的数据比较及监控,是一项有效的推动基层医院输血医学进步的措施;能进一步为县级医院临床输血提供高效、安全的临床管理模式。Objective To understand and evaluate the current situation of clinical transfusion management in one county-level hospital,named Jinggu People′s Hospital.Methods The patients requiring blood transfusion in the same period before(December 2017-May 2018)and after(December 2018-May 2019)the establishment of Quality Control and Monitoring System for Clinical Transfusion in our hospital were classified into control group(n=158)and study group(n=185),respectively.Quality indicators of blood transfusion practices were monitored in terms of the ratio of safe transfusion,the qualified collection of blood specimens,and the qualified application forms of blood transfusion;the differences between the two groups were analyzed via statistical t-test.Results The coincidence rate of indicators used to guide transfusion therapy was 96.22%(178/185)in study group and 80.38%(127/158)in control group;the rate of complete filling in informed consent forms for blood transfusion was 94.59%(175/185)and 82.28%(130/158),respectively;the rate of recording transfusion monitoring was 96.76%(179/185)and 91.14%(144/158),respectively;the rate of post-transfusion evaluation was 89.19%(165/185)and 80.38%(127/158),respectively.Significant differences were observed in forementioned indicators(t=5.518,P<0.01).The rate of standard practice of blood specimen collection was 96.22%(178/185)in study group and 90.51%(143/158)in control group;the rate of standard filling in Blood Components Transfusion Request Form was 95.14%(176/185)and 87.97%(139/158),respectively.Significant differences were observed in forementioned indicators(t=8.822,P<0.01).Conclusion It is an effective measure to enhance blood transfusion medicine in primary hospitals through strict management of blood transfusion and establishment of transfusion monitoring on each procedure.It can further provide an efficient and safe clinical management model for clinical blood transfusion in primary hospitals.
