乌司他丁对老年胃肠道手术患者围术期心脏功能的影响

摘　　要：目的观察乌司他丁在老年胃肠道手术中的应用效果,分析其对患者围术期的心脏功能的影响。方法选取南阳医学高等专科学校第二附属医院普外科2016年1月到2018年12月间的120例老年胃肠道手术患者为研究对象,依据随机数字表法分为观察与对照两组,均60例。观察组患者在开放静脉之后给予泵入乌司他丁,对照组给予硝普钠,观察两组患者围术期的心脏功能指标变化。结果观察组与对照组在手术开始2 h、3 h的ESV、EDV和LVEF以及CO较手术开始前有不同程度的降低,且数据的比较差异有统计学意义(P<0.05)。观察组在手术开始2 h、手术开始3 h的ESV、EDV和LVEF以及CO均优于对照组,差异有统计学意义(P<0.05)。观察组与对照组在手术结束6 h和手术结束12 h的BNP、cTn和CK-MB均较手术开始前有不同程度的提高(P<0.05),差异有统计学意义;且观察组与对照组在手术结束6 h和手术结束12 h的BNP、cTn和CK-MB比较有统计学意义(P<0.05)。结论临床中对于老年胃肠道手术中应用乌司他丁对其围术期的心脏功能起到较好的保护作用,避免心肌损伤,提高手术的安全性,值得临床中应用与推广。Objective To observe the application effect of multinational in senile gastrointestinal surgery, analyze the impact on cooperative cardiac function. Methods 120 cases in our hospital general surgery in elderly patients with gastrointestinal major surgery in January 2016 to December 2018 between the research object, based on randomly divided into observation and control groups, the observation group were in the open veins After giving pumped Ulinastatin control group was given sodium nitroprusside, observed changes in cardiac function index perioperative patient groups. Results The study group and the control group at the start of surgery 2 h, surgery began ESV 3 h of, EDV and LVEF and CO compared with before surgery have a different degree of reduction, and the difference was statistically significant data(P<0.05). In the observation group, the start of surgery 2 h, 3 h of the start of surgery ESV, EDV and LVEF and CO were better than the control group, the difference was statistically significant(P<0.05). Observation group and control group in the end of surgery and surgical end 6 h 12 h of BNP, cTn and CK-MB compared with before surgery have different degrees of increase(P<0.05), the difference was statistically significant;and the observation group and control group the end of surgery and surgical end 6 h 12 h of BNP, cTn and CK-MB was statistically significant(P<0.05). Conclusion The clinical gastrointestinal major surgery for the elderly in their application Ulinastatin perioperative cardiac function plays better protection, worthy of clinical application and promotion.
