2016年四川省医用X射线诊断应用频度调查结果分析

摘　　要：目的通过抽样调查掌握四川省医用X射线诊断的基本情况,研究并估算四川省医用X射线诊断应用频度与分布特点。方法按照国家方案中要求的随机抽样原则,选取四川省21个市州具有一定代表性的1 654家医疗机构进行X射线诊断人次调查。重点选取了7个市州46家开展X射线诊断的机构,采用全国统一的调查表,调查2016-01-01/12-31其全部医用X射线诊断活动,根据医院级别、工作人员数、门急诊人数和住院人数等建立多重线性回归模型估算出四川省医用X射线诊断的应用频度。结果四川省1 654家医疗机构的X射线诊断总人次为19 574 128,其中常规X射线诊断13243 264人次,CT检查5 834 515人次,其他X射线诊断496 349人次。重点调查的46家医疗机构共有X射线影像诊断设备205台,门急诊总人次11 822 965,住院总人数706 626,X射线诊断总人次为2 395 249。样本中受检者男女比例较为平均;年龄分布中">40岁"组占比最高;各种检查类型三级医院的X射线诊断频次占比最高;经估算,四川省2016年医用X射线诊断应用频度约为522.77人次/千人口,CT检查的应用频度约为162.29次/千人口。结论通过本次调查大体掌握了四川省医用X射线诊断应用的基本情况,其应用频度仅低于国内经济发达地区,在全国处于中等水平。X射线诊断检查中,CT检查成为了仅次于常规X射线检查的主要手段,相关部门应加强监督管理,确保X射线诊断工作的正当化。Objective Through the sampling survey to grasp the basic situation of medical X-ray diagnosis in Sichuan province, to study and estimate the application frequency and distribution characteristics of medical X-ray diagnostic in Sichuan Province. Methods According to the random sampling principle required in the national plan,a total of 1654 representative medical institutions in 21 cities and prefectures of Sichuan province were selected for X-ray diagnosis. Focus on the selection of 46 X-ray diagnostic institutions in 7 cities and counties, a national unified questionnaire was used to investigate all medical X-ray diagnostic activities from Jan. 1 2016 to Dec. 31 2016.The application frequency of medical X-ray diagnosis in Sichuan province was estimated by establishing multiple linear regression models based on hospital level, number of staff, number of outpatient and emergency patients and number of inpatients. Results The total number of X-ray diagnoses in 1 654 medical institutions in Sichuan province was 1 957 428 128 cases, of which 13243 264 cases were routine X-ray diagnoses,and 5 834 515 cases were CT examinations and 496 349 cases were other X-ray diagnoses. There were 205 X-ray diagnostic equipment in 46 medical institutions, there were 11 822 965 outpatient and emergency cases, about 706 626 inpatients and2 395 249 X-ray diagnostic cases. The sex ratio was relatively balanced;the above 40 years of age group was accounted for the highest proportion;the tertiary hospitals of various examinations was accounted for the highest frequency of radiological diagnosis;Diagnostic frequencies which were estimated in Sichuan province in 2016 of X-ray diagnosis procedures was about 522.77 per 1 000 population and 162.29 per 1 000 population of CT diagnostic procedures. Conclusion Through this survey, the basic situation of medical X-ray diagnostic applications in Sichuan province has been basically mastered, and the application frequency is only lower than that of domestically developed regions,it is at a medium lev
