摘　　要：目的:回顾性对比分析毒性多结节性甲状腺肿(toxic multinodular goiter,TMNG)131I治疗和手术治疗疗效、预后及费用情况,为临床对本病的治疗方案的选择提供更多的依据。方法:提取我院海泰电子病历系统(V3.0)2010年3月至2014年3月确诊毒性多结节性甲状腺肿并进行手术或131I治疗的病例资料,按照治疗方式分为手术组和131 I组,比较患者病史资料、具体治疗方案、出院诊断及住院费用等。结果:⑴治疗后12个月总有效率:手术组52.63%,131I组87.28%;⑵住院总费用:手术组(11975.45±2987.77)元,131I组(4755.99±1556.32)元;⑶治疗后12个月甲状腺功能减退发生率:手术组94.74%(72/76),131I组20.34%(12/59)。结论:两种治疗毒性多结节性甲状腺肿的方法各有绝对优势,互相不能替代。Objective The therapeutic effect,prognosis and cost of 131I and surgical treatment of toxic multinodular goiter(TMNG)were analyzed retrospectively to provide more basis for clinical treatment.Methods Haitai Electronic Medical Record System(V3.0)of our hospital was used to collect the case data of toxic multinodular goiter operated or treated with 131 I from March 2010 to March 2014.According to the treatment method,the involved cases were divided into operation group and 131 I group.The extracted content included patient history data,specific treatment plan,discharge diagnosis and hospitalization expenses for comparison.Results(1)The total effective rate in 12 months after treatment:52.63%in operation group and 87.28%in 131 I group;(2)the total cost of hospitalization:(11975.45±2987.77)yuan in operation group and(4755.99±1556.32)yuan in 131 I group;(3)the incidence of hypothyroidism in 12 months after treatment:94.74%(72/76)in operation group and 20.34%(12/59)in 131I group.Conclusion The two methods for the treatment of TMNG have absolute advantages and cannot be replaced by each other.
