摘 要:目的了解脑卒中高血压共病患者记忆功能状况,探讨日常生活能力和病前生活方式对其记忆功能的影响。方法选取2018年9月-2019年4月某医院住院治疗的脑卒中高血压共病患者404例,采用Rivermead行为记忆测验第2版(RBMTⅡ)进行测评,同时收集患者一般资料。单因素采用t检验、方差分析;多因素采用多元逐步回归进行数据分析。结果脑卒中高血压共病患者记忆障碍率为88.36%,多因素分析显示,年龄(t=-2.830,P=0.005)、工作情况(t=-2.306,P=0.022)、锻炼频次(t=2.632,P=0.09)、日常交流(t=3.123,P=0.002)、平时遇到问题想办法解决(t=2.938,P=0.003)、日常生活能力(t=-3.097,P=0.002)、口服降压药情况(t=3.162,P=0.002)、脑卒中发病次数(t=-2.751,P=0.006)最终进入回归方程。结论脑卒中高血压共病患者存在明显的记忆障碍,提高日常生活能力并指导患者养成良好的生活方式有助于降低卒中后认知障碍的发生。Objective The study aimed to investigate the status of memory function in patients with comorbid stroke and hypertension and to explore the effects of activity of daily living and premorbid lifestyle on their memory function.Methods A total of 404 cases with comorbidity of stroke and hypertension from a hospital were selected in 2018.9-2019.4.The Rivermead Behavioral Memory Test(RBMTII)was used for evaluation,and the general data of the patients were collected.Single factor analysis of t test and ANOVE,as well as the multivariate stepwise regression was adopted to analyze the data.Results The rate of memory impairment in patients with comorbidity of stroke and hypertension was 88.36%.Multivariate analysis showed that age(t=-2.830,p=0.005),working status(t=-2.306,p=0.022),frequency of exercise(t=2.632,p=0.09),daily communication(t=3.123,p=0.002),encounter problems to solve(t=2.938,p=0.003),activity of daily living(t=-3.097,p=0.002),oral antihypertensive drug(t=3.162,p=0.002),and stroke onset times(t=-2.751,p=0.006)finally entered the regression equation.Conclusion Patients with comorbidity of stroke and hypertension have obvious memory impairment.Improving daily living ability and guiding patients to develop healthy lifestyles can help reduce the incidence of cognitive impairment after stroke.
